LEARNING CONTRACT
To Support Application for Learning Beyond Registration
MODULE No: / MODULE TITLE:South Central SHA provides funding for SUHT to support staff achieving post-qualifying awards and modules provided through the University of Southampton, Faculty of Health Science. Attendance and achievement data is monitored by the Faculty and reported back to the Care GroupEducation Leads and Nursing and Midwifery Group on a quarterly basis.
There is a requirement for applicants accessing these funded places to attend study days and to submit the summative assignment at the end of the module (regardless of whether credit is required) as the knowledge and skills/competency are required for job role.
Listed below are the required lines of communication throughout the duration of your module studies. It is the responsibility of you as an individual to contact these people to inform them of any problems or concerns.

Please Note: If you fail to submit a module assignment, you should be aware of the following:
- No Funding will be made available to you for any study days, conferences etc that are not classified as Mandatory or a necessity for your clinical practice for the period of a full year.
- Study Days, conferences etc that you apply for over the next two years will be reviewed by the Lead Nurse Team before being granted.
- Results also appear on The Learning Manager database, which is used as part of the appraisal process at ward level with your line manager.
- If you do not follow education and trust processes for non submission it may result in disciplinary action.
SUHT strives to support you in the attainment of knowledge and training and these processes have been put in place to help you and reduce the number of wasted spaces that in turn make it harder for individuals to access courses.

Part 1. To be signed by applicant.
(a) I understand that in applying for this module / award pathway, that I am required to attend all of the taught study days associated with this module and that failure to attend the full study days without a valid reason, will result in my not being financially supported with any further academic modules or programmes of study.
(b) I understand that in obtaining funding support for this module I am required to undertake the summative assignment at the end of the module whether or not the academic credit is required. Failure to submit assignments will result in my not being financially supported with any further academic modules or programmes of study.
(c) I understand that I am required to report back the results of my assignments to my line manager and to my care group education lead as soon as I am notified of them.
(d) I have read and understood the mechanism for reporting any problems that I may have whilst undertaking module, and agree to report to relevant persons and comply with the education provider process for applying for deferral.
(e) I have read and understand the impact of failure to adhere to the above terms will result in further applications for funded study, conferences being refused and that failure to adhere to the above may result in disciplinary action.
Part 2 -On submission of an assignment you will provide a summary of learning which has occurred and recommendations for practice to your ward manager and education lead.
For research modules proposals:- you will met and discuss with education lead at planning phase to ensure that your chosen project meets both service needs as well as personal interest

(c) I have discussed the expected module outcomes with the applicant and will endeavour to help them and/or refer him/her to the Care Group Education Lead should they be referred on their assignment at first attempt.
