4)Supervisor declaration about academic links or contacts in the Foreign Institution regarding the planned thesis projects,

5)Copy or valid ID or passport.

DECLARES

A)Of being aware he/she will be required, when being told of having been endowed with the grant, to hand over to the Head of the DICAM:

1)How he/she would like to receive the grant (bank account details)

2)Form detailing personal and fiscal data

3)Communication of exact leaving date

B) Of having sufficient knowledge of the language spoken in the country of destination or English language.

C) Of not having benefited in the past from a scholarship for thesis abroad

D) Of having been assigned a scholarship for the same purpose in the past but having renounced in the following date …………………. (wipe out the one not applicable)

E) Not meeting anyone of the incompatibility circumstances detailed in the Call
