Service Agreement

Convenience Driving School, hereinafter referred to as “Driving School” agrees to provide applicant, hereinafter referred to as “Student”, or “Client” for rehabilitation portion of contract.

ð  Classroom Only, Three Times Failed (under 19 years of age) Package, $275 ($284.63) - 36-Fifty (50) minute in-class Driver Education course. Student 18 years of age or younger who have not completed In-class Driver Education and failed DMV Knowledge test Three Times, must complete this course to take DMV exam a fourth time. Once Student has completed the course, the Student will receive certificate of competition*

ð  Complete Driver Education Package (Under 18 years of age) Package, $475 ($491.63) - 36-Fifty (50) minute lessons in-class instruction, seven (7)-fifty (50) minute observation and seven (7)-fifty (50) minute in-car behind-the-wheel driving instruction; on the seventh lesson in-car Student will take the road test with Driving School. Once Student completes the on-road portion and passes DMV road test Student will receive certificate of completion.*

ð  Behind the Wheel (under 18 years of age) Package, $300 ($310.50) __ Special $250 ($258.75) – 7-fifty (50) minute in-car behind-the-wheel lessons and seven (7) fifty (50) minute in-car observing another Student driving; on the seventh lesson in-car Student will take DMV road test. Once Student completes the on-road portion and passes DMV road test Student will receive certificate of completion.*

ð  Three Times Failed Road Package, $300 ($310.50) - 7-fifty (50) minute sessions in-car behind-the-wheel driving instruction. Once Student completes the on-road portion and passes DMV road test Student will receive certificate of completion.*

ð  Three Time Failed Knowledge (Any age, Completed In-class Driver Ed) Package, $175 ($181.13) – Eight (8) hour classroom course based on the Virginia Driver's Manual. Once Student has completed the course, the Student will receive certificate of competition*

ð  Test Prep Package, $100 ($103.50) – Student will receive (4) four hours of in-class instruction on Virginia Driver Manual to prepare Student for DMV knowledge test.*

ð  Wavier Program, driving portion only (18 years of age or older) Package, $325 ($336.38) - Students who are 18 years of age or older waive the 60 day holding period and the DMV road skills test to receive their driver license. Student will complete seven (7) fifty (50) minute instruction in-car behind-the-wheel instruction seven (7) fifty (50) fifty minute sessions observing another Student drive; on the seventh lesson in-car Student will take DMV road test. Once complete Student receive certificate of competition which DMV will issue driver license.*

ð  Waiver Program, classroom and driving portion (18 years of age or older), $525 ($543.38) - Students who are 18 years of age or older waive the 60 day holding period and the DMV road skills test to receive their driver license. Student will complete thirty-six (36) and fifty (50) fifty minute In-Class Driver Education instruction, and complete seven (7) fifty (50) minute instruction in-car behind-the-wheel instruction seven (7) fifty (50) fifty minute sessions observing another Student drive; on the seventh lesson in-car Student will take DMV road test. Once complete Student receive certificate of competition which DMV will issue driver license.*

ð  Wavier Program, quick program (18 years of age or older) Package, $750 ($776.25) – Students who are 18 years of age or older waive the 60 day holding period and the DMV road skills test to receive their driver license. Student will complete thirty-six (36) and fifty (50) fifty minute In-Class Driver Education instruction, and complete seven (7) fifty (50) minute instruction in-car behind-the-wheel instruction seven (7) fifty (50) fifty minute sessions observing another Student drive. Once complete Student receive certificate of competition which DMV will issue driver license.*

ð  Driving Lessons (Hourly Reduction Rate), $40/hr. ($41.40) – Student will receive one-on-one driving lessons in Driving School’s vehicle with a licensed instructor. The Student can receive any amount of training based upon the needs and determined by the person/organization paying for services. The Driving School offers a discount for long periods of driver training. Students requiring pick up/drop off complies with Pick up/Drop off Package, price will be added. [Discounts: 6.25% for eight (8) hours to twenty (20) hours; 16.7% for thirty (30) hours to fifty (50) hours]. *

ð  No Show, $25 ($25.88) – When Driving School has a Student scheduled to drive and the Driving School begins the process to pick up Student, or travels to the location to pick up Student and Student does not show or drive; a NO SHOW fee will be charged. The NO SHOW fee and if PICK UP/DROP OFF PACKAGE complies, both fees will be assessed for a NO SHOW. *

ð  Pick up/ Drop off Package, $10 ($10.35) – The Driving School will make arrangements to pick up and drop off Student at an agreed location which is not a normal area the Driving School picks up and drops off Students. The Student will pay the fee for each day the service is needed. If the fee covers an area within fifteen (15) minutes of Driving School’s normal area. The fee covers two way travels; the time is one way travel for each day travel is required. The Student/Organization agrees to pay an additional fee for every fifteen (15) minutes of travel. [i.e. Travel of 25 minutes will be charged ten ($10) dollars].*

ð  Special course (teaching 8 hours or less), $100 ($103.50) - Students who need/want to complete a class, course, program, package at a special scheduled time; the Driving School will schedule the class, course, program, or package to meet the Students and Driving School schedule. The Student will be charged an additional fee. DOES NOT APPLY TO CLASSES LONGER THAN HOURS.*

ð  Special course (teaching more than 8 hours), $200 ($207) - Students who need/want to complete a class, course, program, package which is longer than 8 hours of teaching at a special scheduled time; the Driving School will schedule the class, course, program, or package to meet the Students and Driving School schedule. The Student will be charged an additional fee.*

ð  Use driver education car to take road test at DMV, $150 ($155.25) - Student will be taken to the DMV to take their road skills test. Student will use Driving School car to take DMV road test, then Student will be brought back to agreed location.*

ð  Fuel Surplus Fee, ($2.00 x ______= $______) - Upon the date of agreement, if the price of gasoline is $3.50 or greater at location Driving School purchases fuel, a fuel surplus fee will be added in addition to the cost of the package, course, or class cost. The fuel surplus fee is $2.00 per lesson [per fifty (50) minutes of in-car].

ð  Medical Testing/Evaluation $375 ($388.13) – Client will receive Medical Testing/Evaluation given by an ADED Certified Driver Rehabilitation Specialist (CDRS). The Client must have all required documents, complete all required paperwork, and Driving School must follow directions and rules of the Virginia DMV Medical Review. The CDRS will determine the best way to evaluate the client and required to share all information with Virginia DMV. *

ð  In-class Training for Client with Intellectual Disability, $40/hr. ($41.30/hr.) – Client with intellectual disability will be taught in-class instruction with one or two licensed instructor and/or assistant. If Client requires one-on-one instruction during the scheduled class, the Driving School will provide an assistant to help the Student.*

ð  In-car Training for Client with Intellectual Disability, $50/hr. ($51.75/hr.) – Client with intellectual disability will be taught in-car behind-the-wheel with a licensed instructor. Student with intellectual disability will receive instruction in-car with a licensed instructor. *

ð  In-car Training for Clients with 3-W Restriction, $110/hr. ($113.85) – Clients with a 3-W restriction are required to perform driver training with CDRS and a Driver Education Instructor. Client will receive training in car with all required equipment to train Client for safe operation of vehicle, safe use of any equipment required, other safety instruction, and prepare Client for DMV road test. Upon competition of training the DMV will be notified to schedule road skills test. *

ð  In-car Training for Client with Adaptive Equipment, $80/hr. ($82.80) – Client which required adaptive equipment for training will receive training with the required adaptive equipment and a licensed instructor. Client will be taught based upon the instruction of Certified Driver Rehabilitation Specialist (CDRS). Training will include teaching safe use of adaptive equipment, safe driving with adaptive equipment, other required training, and preparing Client for DMV roads skills test. *

ð  Take Student to Woodrow Wilson Hospital, $30/hr. ($31.05/hr.) – Client will be picked up at scheduled location. Client will be taken to Woodrow Wilson Rehabilitation Center; if requested Client will be brought back to agreed location. Cost will begin when Driving School’s vehicle leaves office until Driving School’s vehicle returns to Driving School. *

ð  Take Client with Adaptive Equipment to DMV for Road Test, $250 ($258.75) – Client requiring adaptive equipment will be picked up, or meet at DMV, to use Driving School vehicle with the required adaptive equipment to take DMV road skills test. If required Client will be returned to agreed location. If Client requires transportation which complies with pick up/drop off package, price will be added. *

ð  Forty-Five (45) hour Manual Package, $1500 ($1,552.50) - Student will be driven fourth-five (45) hour total which is thirty (30) daytime hours, and fifteen (15) after sunset hours based on the DMV forty-five (45) hour manual. Student will be taught all lessons within the guide. (If Student requires pick up/drop off, the price will be determined before entering agreement). *

ð  Driving Lessons for Intellectual Client (Hourly Reduction Rate) $45/hour ($46.58) - Client will receive one-on-one driving lessons in Driving School’s vehicle with a licensed instructor. The Client can receive any amount of training based upon the needs and determined by the person/organization paying for services. The Driving School offers a discount for long periods of driver training. Clients requiring pick up/drop off complies with Pick up/Drop off Package, price will be added. [Discounts: 6.25% for eight (8) hours to twenty (20) hours; 16.7% for thirty (30) hours to fifty (50) hours]. *

*Students or Clients 18 years of age or older must drive or observe with Students or Clients 18 years of age or older. At no time can a Student or Client 18 years of age or older receive instruction or observe with Students or Clients younger than 18 years of age in Driving Schools vehicle.

* When Student turns 18 years of age, the temporary license can no longer be used with learner permit as a valid driver license. Student must take temporary license to DMV to receive a driver license.

* Student’s license will expire on eighteen (18th) birthday. Student must take paper issued by Driving School to DMV Customer Service on or after eighteen (18th) birthday to receive a valid driver license. When Student becomes eighteen (18) years old driving with paper license issued by Driving School and learner permit; the Students license is expired until Student receives license from DMV, or hard copy of license.

*Meets or exceeds standards set by the Virginia Department of Education (DOE) and Virginia Department of Motor Vehicles (DMV).

* Student must hold valid learner permit to complete in-car behind-the-wheel instruction

*Student has no guarantee to pass any package, course, class, or program offered by Driving School.

*Student under medical review must comply with DMV Medical Review instruction to receive driver license.

+All prices are cash, check, or credit/debit card is charged a 3.5% processing fee for and package, course, class, program, or fee. The price of each package, course, class, program, or fee is listed. The price in brackets ($__ . __) includes the processing fee.

+Fuel Surplus Fee will be added in addition to the cost on any package, course, class, program we offer in which the driver education car is used and the price of fuel is $3.50/gallon or higher. The Fuel Surplus Fee is $2.00 per lesson or per hour.

+No Show fee will be assessed to any Student or Client who does not show or does not drive when scheduled based upon the No Show and Missed Class portion of the contract. An additional fee of twenty-five ($25) dollars will be added to the cost of the package, course, class, and program each time of occurrence, processing fees apply.

The Driving School is licensed by the Virginia Department of Motor Vehicles to provide driving classes, and licensed by the Virginia Department of Motor Vehicles Medical Review to provide testing, evaluations, and training rehabilitation services. The Driving School has a Certified Driver Rehabilitation Specialist (CDRS) and instructors certified and licensed in many fields and organizations to provide the best possible services to our Students and Clients.

All classes meet or exceed the Standards of Learning (SOL) for Virginia Department of Education (DOE), and the Virginia Department of Motor Vehicles (DMV). The minimum number of classes required to complete classroom instruction for students under 18 years of age is 36 class periods. The minimum number of periods required to license a student under 18 years of age is 7 behind-the-wheel, and 7 periods of in-car observation. Student age 18 or older are not required to complete Driver Education to receive a driver license in Virginia.

The State of Virginia requires the Student to obtain a proper learner's permit from the Virginia Department of Motor Vehicles (DMV). The Student may begin classroom instruction before obtaining a learners permit. However, the Student is required to obtain a valid learners permit, all required forms completed, and pay tuition in full prior to driving behind-the-wheel driving portion of the training. The Student MUST have their learner’s permit in their possession during all behind-the-wheel practical instruction.

Virginia Department of Motor Vehicles (DMV) does not require driver education training for students who are 18 years of age, or older student must hold learners permit at least 60 days to receive a driver license. Student 18 years of age or older can complete a driver education course, and then student can receive driver license immediately upon passing all required DMV test.
