Purpose:

FANTA is seeking price quotations from qualified vendors to provide printing services for the printing of Body Mass Index (BMI) Calculators, called BMI Wheels. The wheels will need to be printed in French, English and Portuguese.

The BMI wheels will be used for training purposes, routine nutrition assessment, counseling and support services at health facilities (clinics) and community nutrition screening activities.

Background:

Food and Nutrition Technical Assistance III Project (FANTA) is a USAID-funded project to improve nutrition and food security policy, strategy and programming, and provide technical support to USAID and its partners, including host country governments, international organizations and other implementing partners. FANTA develops tools and adapts approaches to support the design and quality implementation of field programs, while building on field experience to improve and expand the evidence base, methods, and global standards for nutrition and food security programming.

Since 2010, FANTA has been working with Boston Children’s Hospital to develop a body mass index calculator tool designed to categorize the nutritional status based on weight and height ratio. The tool has been successfully used by FANTA in various countries.

On February 1, 2016 FHI 360 signed the Content Use Agreement with the Boston Children’s Hospital, enabling FANTA III (as Licensee) to use, make, copy, reproduce, publish, display, distribute, transmit, make derivative works, modify, adapt, revise, expand, and translate the content of the BMI Wheel (model attached) for project needs.

REQUEST FOR QUOTATIONS

1. Technical Specifications:

Print upon request BMI Wheels which can be in any of three current languages (English, French, Portuguese – see attached images for reference) in the following quantities:

Please also provide your cost to set-up the artwork for all three languages.

Printing Specifications:

Each BMI Wheel is comprised of three circles:

o  Middle 8 inch

o  Front 7.25 inch

o  Back 7.625 inch

-  Full color

-  White plastic with matte finish and grommet in the middle

-  Thickness of the opaque whiteplastic.020”

-  Artwork EPS files will be provided

3. Delivery/Shipping Requirements

The selected vendor will need to package the wheels for shipment to several locations around the world.

Shipping costs will be covered by FHI360/FANTA through direct billing.

FANTA will provide the selected Vendor with the shipping account and with the shipping addresses for the delivery, with each printing Order.

4. Minimum Requirements:

Fulfillment of all the specifications listed above.

5. Evaluation Criteria:

Selected vendor will be based on lowest price that meets all the specifications and quality.

6. Payment Terms:

Quotes should be provided in unit price and should include payment terms.

7. Submission Requirements

All materials produced and delivered by the vendor, and all work products, will be considered “work for hire” and will become the exclusive property of FHI 360. The vendor must be prepared to warrant that all deliverables will be free of material faults and errors and that no copyrights have been violated.

8. Response

Responses to this RFQ should be submitted by email to Aferdita Sadiku () by June 21st, 2016 COB. Any quotes received after this date and time may not be accepted and shall be considered non-responsive. FHI 360 will acknowledge receipt of the quotation by return email.

Any questions or requests for clarification need to be submitted in writing to the same email address as above by June 20th, 2016. Inquiries and answers to inquiries will be shared with all applicants. No telephone inquiries will be answered.

Submitted quotes should be valid for at least 60 days from the day of submission to allow adequate time for organizational approval.

9. Contracting Mechanism:

A fixed-price purchase order may be awarded in U.S. dollars (USD) to the responsive vendor whose quote will be evaluated as most advantageous to FHI 360. In the event that items amounts need to be shifted during the time period between the signature of a purchase order with the selected vendor and the actual event, FHI 360 will modify the purchase order.

NOTE

1) FHI 360 will not compensate vendors for preparation of their response to this RFQ.
2) Issuing this RFQ is not a guarantee that FHI 360 will award a purchase order.
3) FHI 360 reserves the right to issue a purchase order based on the initial evaluation of offers without discussion.
4) FHI 360 may choose to award a purchase order for part of the activities in the RFQ.
5) FHI 360 may choose to award a purchase order to more than one vendor for specific goods in the RFQ.
6) FHI 360 may request from short-listed vendors a second or third round of written response to a more specific and detailed scope of work that is based on a general scope of work in the original RFQ.
7) FHI 360 has the right to rescind an RFQ, or rescind an award prior to the signing of a purchase order due to any unforeseen changes in the direction of FHI 360’s client (the U.S. Government), be it funding or programmatic.

8) FHI 360 reserves the right to waive any deviations by vendors from the requirements of this solicitation that in FHI 360’s opinion is considered not to be material defects requiring rejection or disqualification; or where such a waiver will promote increased competition.

REQUEST FOR PROPOSAL FIRM GUARANTEE

All information submitted in connection with this RFQ will be valid for sixty (60) days from the RFQ due date. This includes, but is not limited to, cost, pricing, terms and conditions, service levels, and all other information. If your firm is awarded the contract, all information in the RFQ and negotiation process is contractually binding.

OFFER VERIFICATION

FHI 360 may contact offerors to confirm validity of the offer, contact person, address, and bid amount.

FALSE STATEMENTS IN OFFER

Offerors must provide full, accurate, and complete information as required by this solicitation.

CONFLICT OF INTEREST

Offerors must provide disclosure of any past, present, or future relationships with any parties associated with the issuance, review, or management of this solicitation and anticipated award. Failure to provide full and open disclosure may result in FHI 360 having to re-evaluate selection of a potential offeror.
