/ Ø Appointments preferred to secure your spot, bring your confirmation number on day of clinic. Walk-ins will be accepted as schedule allows

Ø MUST bring insurance cards – Medicaid, NC Health choice, or other private insurance.
Ø Certain Cigna and Blue Cross Blue Shield plans will be billed
Ø Payment may be required with some insurance plans.
Ø Be prepared to wait and plan accordingly (You might have to wait outside, rain or shine)
Ø MUST bring official immunization records and at least ONE form of ID
/ Ø Kindergarten through Fifth Grade (K-5) health assessments
Ø Free vision, dental, and health screenings
Ø Free entertainment and giveaways
Ø Referrals for further medical care
Charlotte Mecklenburg Schools Exclusion date is September 28.
Food and drinks are not available at either event – pack what you will need.
