Re: Athletic Participation for all sports
Your child’s participation in our athletic program will afford them the opportunity to participate in the Carmel tradition of excellence in both academics and athletics. Our student athletes are provided an opportunity to grow in character, physical fitness, sportsmanship, and life skills. These qualities enable each athlete to become a contributing member of the Carmel Family and our greater community.

We will also have some available in the front office.
Each form must be read and properly completed prior to the first day of try-outs. Please do not leave any part of the forms blank. All forms must be completed and returned to Carmel before a student may attend try-outs. All sports offered at Carmel are on a try-out basis.

Please view, complete, and RETURN the following forms during sign-ups.
1. Middle School Student-Athlete Pre-Participation Forms: Complete the Medical History on the Physical sheets (yes/no questions), have both parent and student signatures, and have a licensed physician complete the Physical Examination Sheet. Carmel Middle will provide a ONE-TIME opportunity for students to get physical at Carmel Middle on Friday, September 2nd for $25, CASH ONLY. This physical will be valid for one year and are provided by Novant Health.
2. Insurance Form – Green form (football) or Blue form (all sports except football). Provide policy number.
3. Concussion Form - Both parents and students need to initial and sign.
4. Play Fair form – Both Parents and students need to initial and sign
5. Optional - Cougar Athletic Club form. Thank you for supporting our athletic program.

Carmel Athletic Booster Club would like to invite you to become an active member of the organization. The Carmel community has been most supportive of our fine athletic programs.
We hope that all students, parents, and members of the Carmel community will join this organization. The Carmel Athletic Booster Club believes a well-rounded athletic program is a valuable part of the overall educational experience. Our club supports the school administration, the athletic department, and the coaching staff in whatever way possible to enhance the Carmel Middle Athletic program throughout the school year. With your continued support, the Carmel Athletic Booster Club can continue upgrading athletic equipment, recognizing all student athletes at awards programs, providing transportation to and from athletic events, and placing a major emphasis on academic achievement for all our student athletes.
A suggested donation for membership in the Carmel Athletic Booster Club is $35.00 per family. Checks may be made payable to Carmel Athletic Booster Club. Thank you in advance for your willingness to support athletics at Carmel. Cougar Pride!

Sports Physicals – Novant Health
Carmel Middle School
Friday, September 2nd
$25 – CASH ONLY
Novant Health Sports Medicine doctors will be on campus on Friday, September 2nd to provide sports physicals for $25. These physicals will be good for one year and can be used for 7th and 8th graders to complete the required physical requirement to try out for any sport at Carmel. They could also be used for summer camps or other needs throughout the year. This will be the ONLY time physicals will be provided at Carmel. Please bring $25 cash and your physical form to school Friday to receive a sports physical. Parents must complete the Medical History part and sign that you give permission for the physical.
