GUIDANCE FOR APPRENTICES – COMPLETING A PMC FORM
IT IS YOUR RESPONSIBILITY TO STATE YOUR CASE CLEARLY AND SUPPORT IT WITH RELEVANT AND APPROPRIATE EVIDENCE. RE-SUBMISSIONS OF PMCS ARE NOT PERMITTED.
- If you decide to submit a PMC form make sure that you complete the form yourself and submit it to . The form will be considered by the PMC panel.
- Make sure that all sections of the form are completed.
- Identify in detail the assessment(s) affected (e.g. an interview, an examination, a test, a research project, a performance, an oral examination).
- Attach documentary evidence to your PMC form; PMC claims are not considered if independant relevant evidence is not submitted (ask yourself ‘what would convince me if I was considering this PMC?’)
Mitigation Claimed For
Indicate for each assessment whether you are claiming mitigation for absence, late submission, non-submission or standard of performance (you can only claim for one of these for each assessment).
- Absence: absence from an examination, presentation, set task, etc.
- Late submission:(up to and including 14 days). The assessment was/will be submitted up to 14 calendar days later than the assessment deadline date.
- Standard of performance:you have submitted the piece of work by the deadline but the quality of what was submitted was ‘beyond your control’. A claim of this type has to be completed before the grades of the assessment are released. Once the grades are released mitigation cannot be claimed for.
- Non-submission:nowork not submitted at all.
Important – Reasonable Adjustments
Apprentices who have reasonable adjustments which covers individual arrangements for assessments and examinations do not need to complete a PMC form to take up these arrangements.
However, if apprentices wish to claim for something over and above what is covered by the reasonable adjustment, they must complete a PMC form. For example, an apprentice who has an automatic 1-week extension to assessed work deadlines but then suffers a severe bout of food poisoning which requires hospitalisation for 1 week. Alternatively, an apprentice may wish to claim for a worsening of the symptoms covered by a reasonable adjustment. For example, an apprentice may find that arthritis in his/her hands, which is covered by an automatic extension, is worse than usual because of a change in medication and therefore he/she needs further arrangements. If this is a temporary situation, eg whilst medication is stabilised, the claim should be made on a PMC form outlining why this is not covered by the reasonable adjustment.
