Office Visit Information

Preparing for Your Office Visit

Thank you for choosing Gastroenterology Associates of Chattanooga, PC.

Your health is our primary concern. Our providers and staff are committed to providing you with high quality care during your visit. We take steps to make sure your appointment at GAC is positive and convenient. It is important that we have current information for your visit. If you are a new patient or it has been greater than a year since your last visit please take time BEFORE you arrive to PRINT off the Patient Registration form and the 2 page Medical History / Review of Systems form found in this section, in the following pages.

Our Physicians ask that you FILL OUT these forms before you arrive so it does not interfere with the time your provider has to spend with you. Please do not fax your paperwork; we need to review the originals with you. Be sure to bring a current list of medications you take regularly and as needed. To ensure you are able to spend the entire allotted appointment time with your provider, we ask that you arrive 20 minutes prior to your scheduled appointment. This allows our front office and medical assistants to prepare your chart for our providers.

Our front desk will confirm your registration and insurance information at each visit. Our medical assistants will obtain your vitals, update your medication list and review any medical history that may have changed since your last visit.

If you are unable to arrive at the requested time your provider may ask you to reschedule. You will be informed if your physician has had an unexpected delay or an emergency. Please bring your active insurance card and the co-pay required by your insurance plan to each visit at GAC.

We recommend that you check with your insurance company if you have any questions about your co-pays, deductibles or out-of-pocket expenses. We will not

do any procedures in the office during your visit. It is not necessary for you to fast or prepare in any way for an office visit.

CANCELLATION POLICY: A 48 hour notice is required if you need to cancel your appointment. You will be charged $35.00 per appointment if you do not notify us. You may be denied the opportunity to reschedule your appointment until this amount is paid.

HIPAA (Privacy Policy) You may request a copy of the HIPAA Regulation. It is also visible in the office waiting room.
