Applications are invited for the above post in Respiratory Medicine, based at Glasgow Royal Infirmary (GRI) and Stobhill Ambulatory Care Hospital (SACH). This post will commence in August 2016 and is outside the MTAS process.

The Difficult Asthma Service is a Tertiary Referral service for the North-East Sector and Lanarkshire. There is an active Multidisciplinary Team including 2 Consultants, a GP with a Special Interest, a Respiratory Clinical Nurse Specialist and a Specialist Respiratory Physiotherapist with support from the large clinical physiology team based both in GRI and SACH. There are close links with the GRI Clinical Research Facility and also with other Difficult Asthma Services across the city. The Difficult Asthma Service includes a weekly clinic for review of the large cohort of patients and also a weekly clinic for administration of omalizumab injections. We expect to be able to provide treatment with the novel anti-IL-5 antibody, mepolizumab, later this year.

Our active clinical research program has an expanding portfolio of activity with interests including identification of strategies to improve treatment adherence in the difficult asthma population, development of non-steroidal treatment options, the association of vitamin D with asthma, the potential role of pulmonary rehabilitation in this cohort, and the use of biomarkers in treatment rationalisation. We contribute to the UK wide BTS Difficult Asthma Registry and are participating in the UK Refractory Asthma Stratification Programme, a multicentre, MRC-funded project. Other research activities include participation in multi-centre trials both commercial and non-commercial. This new post has been developed to offer training and research opportunities in difficult asthma and respiratory medicine, whilst providing clinical service support both to the expanding difficult asthma service and to general medical and respiratory services within GRI.

Candidates would preferably be in possession of MRCP (UK) or equivalent, have appropriate experience in General and Respiratory Medicine ideally including some experience in management of difficult asthma; although these are not essential requirements and training will be provided in post. Applicants should be planning to continue in a career in Respiratory Medicine or a related specialty.

We would welcome applications from doctors at level CMT2/ST2 or above looking for 2 years research break to consolidate their training, or from senior respiratory trainees considering a pre or post-CCT senior fellowship with subspecialty training.

Clinical duties will include participation in the difficult asthma service, as well as general respiratory inpatient and outpatient work, and general medical on-call.

The post-holder will also be given the opportunity to undertake a period of supervised clinical research, leading to the award of an MD or other higher degree.

Funding is available for 2 years, however shorter-term appointments may be considered.

To practice medicine in the UK you need to hold both GMC registration and a license to practice.

ABOUT US

The Acute Services Division is the largest group of adult acute hospitals in Scotland. It enjoys close links with Glasgow’s three universities and makes a significant contribution to teaching at both undergraduate and postgraduate level. Research also has a high profile within the organisation. Education facilities are provided at Glasgow Royal Infirmary and the new ambulatory care hospital at Stobhill Hospital. The service in North Glasgow presently employs more than 14,300 staff serving a core catchment population of 560,000.

Glasgow Royal Infirmary (GRI) & Stobhill Ambulatory Care Hospital (SACH)

Glasgow Royal Infirmary is one of the major teaching complexes of the University of Glasgow. It provides the Emergency Medicine service for the North Eastern districts of Glasgow and has in-patient beds in general medicine and related specialities, medicine for the elderly, general surgery, orthopaedics, plastic surgery and obstetrics and gynaecology. There are also beds in intensive care, medical and surgical high dependency, and coronary care. Following the closure of Stobhill Hospital March 2011 all acute medical beds, with the exception of some long-stay care of the elderly beds, are on the GRI site. The Stobhill Ambulatory Care Hospital is a modern purpose built ACH providing a full range of out-patient and ambulatory care services including an ENP led Minor Injuries Unit (MIU). Radiology, Cardiology and Respiratory diagnostic services are provided both at GRI and SACH.

Staff at GRI and SACH are proud of the close inter-departmental links and co-operation. They are at the forefront of a progressive agenda in relation to clinical standards, managed clinical networks and patient focused service re-design. There are close clinical links with colleagues in the Community Health Care Partnership and other hospitals in NHS Greater Glasgow and Clyde, Glasgow, Strathclyde and Glasgow Caledonian Universities. There is a dedicated hospital wide academic programme supported by a strong service educational commitment. GRI and SACH are provided with dedicated education centres and IT support. Both have excellent reputations in supporting and nurturing its clinical staff and also have an excellent reputation for under and post graduate training. There are academic units in Anaesthetics, Cardiology, Human Nutrition, Medicine, Surgery, and Obstetrics and Gynaecology.

At GRI medical emergencies are admitted via an Acute Assessment Unit (AAU) run by acute physicians or via the Emergency Department (ED) to an Acute Medicine Unit (AMU). This comprises 4 geographically defined ward areas covered by specific specialty teams – general medical, respiratory, gastroenterology and medicine for the elderly. Where bed availability permits, patients with specific conditions are triaged to the appropriate specialty team e.g. patients with respiratory disease to the respiratory area. Four consultant physicians representing each speciality group take part in acute receiving each day in the AMU with morning and evening ward rounds.

Acute admissions to medicine range from 45 to 70 per day. Patients requiring a short stay are discharged from AMU. Those requiring longer stays are transferred ‘downstream’ to medical, care of elderly or cardiology beds. There are 216 downstream medical beds split into specialty units/wards; Respiratory Medicine, Rheumatology, Gastroenterology, Diabetes and Endocrinology. Consultants from the department of medicine for the elderly (DOME) contribute to receiving duties, but also have 183 in-patient beds in a number of wards within Glasgow Royal Infirmary with additional rehabilitation beds at Lightburn Hospital and Stobhill Hospital. There is a separate Stroke Ward.

The Department of Respiratory Medicine occupies 4 wards in the Old Block (ward 2 on 1st floor, wards 6 and 7/16 on 3rd floor, and ward 10 on 5th floor), in total 67 beds, and additionally operates 16 beds within the speciality-based medical receiving unit.

In addition to contribution to the care of general medical and general respiratory patients, the consultants contribute to most major respiratory subspecialities, including lung cancer, pleural diseases, complex sleep and ventilation, interstitial lung disease, COPD, asthma and bronchiectasis, TB and other respiratory infection.

Both GRI and SACH have pulmonary function laboratories on site that provide standard lung function, bronchial challenge, cardiopulmonary exercise testing and respiratory muscle testing.

Out-patient activities occur at both GRI and SACH, and day case activity is possible within the gastro day unit.

3. Duties of the Post

This post is for 2 years, although 1 year appointments will be considered.

The post-holder’s clinical duties will include participation in general respiratory and difficult asthma clinics (both SACH), a weekly respiratory ward round and seeing inpatient respiratory referrals (referrals page shared with respiratory STs). Supervision will be provided by Dr Cowan and colleagues.

The post-holder will participate in middle grade general medical on-call duties at GRI on a 0.5 wte basis.

. There will be access to the Scottish joint deanery training programme for Respiratory Medicine.

The post-holder will be expected to assist with the day-to-day running of ongoing in-house clinical studies. There will be ample time to undertake a period of supervised clinical research, leading to the award of an MD or other higher degree. The specific research project will be open to discussion with the appointee.

4. Study and Training

Study leave within the United Kingdom will be in accordance with the terms and conditions of service. For overseas study leave, application is made to the Divisions Clinical Director or Associate Medical Director.

An excellent and comprehensive medical library is sited in the hospital and includes a CD-ROM based reference search system. The post holder will be encouraged to attend and participate in the various post-graduate educational forums in the West of Scotland, the Infirmary itself and the Royal College of Physicians and Surgeons of Glasgow.

The trainee will be encouraged to participate in the teaching of medical, nursing and other health care professionals within the unit. There will be opportunities to contribute to undergraduate teaching of medical students from the University of Glasgow.

Research training will be directly provided, including Glasgow Clinical Research Facility modules and University of Glasgow generic training programme. In particular, the post-holder will be encouraged to completed Good Clinical Practice training early in the post.

Participation in audit within the unit is strongly encouraged.

The appointment is full-time, for 2 years. Accommodation is not available.

In terms of NHS Circular 1989 (PCS) 32 dealing with Medical Negligence, the Division does not require you to subscribe to a Medical Defence Organisation. Division indemnity will cover only Trust responsibilities.
