Group Medical Questionnaire / A signed GMQ is required PRIOR to obtaining a quote from the carrier.
Census / Include gender,date of birth, date of hire, coverage election (include waiver reasons), current plan election and residence zip code. Please note that Aetna requires First and Last name, DOB, and zip code of all people to be covered including dependents.excel spreadsheetrequired
Average Payroll Calculation from Prior Year / Average Payroll Calculator – To includefull-time, part-time and seasonal employee count for all commonly owned companies from PRIOR CALENDAR YEAR
Most Recent Prior Carrier Billing / Must include the summary page reflecting the premium paid to date.(Aetna requires this with the RFP)
Copy of Current Carrier Plan Design / Required by all carriers (detailed plan summary)
Current Carrier Rates / If current rates for ALL dep categories are listed on the prior carrier billing this is not necessary
Current Carrier Renewal Rates / Underwritten rates can not be provided without a renewal
Claims Experience/History / required for 100+ groups
5 Year Carrier History / Please list the number of years covered with each carrier. If less than 5 years, please list carriers prior to the current.
Commission Level Requested / Will affect rating so we must have this prior to quoting for all carriers.1% must be built in for all BCBS large group requests 51+ and for all Aetna 100+ size groups

Large Group Participation Requirement (rateload will apply if 50%is not attained):

Aetna = 51-99- 70% participation excluding valid waivers. 100+ loads applied for less than 50%

BCBS = 50% overall participation

Cigna = 50% overall participation (regardless of waiver reason)

Humana = 50% overall participation (regardless of waiver reason)

*benefit specific coverage (i.e. cancer only) are not valid waivers.

Cigna = 20-250 covered medical lives
