The Pikes Peak Interagency Transition Team (PPITT) was created in 1992 when Transition Services were mandated by th e State, and Transition Coordinators needed to tell families what resources were available. The PPITT has evolved so that agencies and providers can stay updated, connected and to network. PPITT also publishes an annual resource directory and hosts an annual resource fair. In addition, PPITT has a listserve where events, etc. are posted and forwarded by members.

Community Intersections

Terry Holmes
7195746101
Community Intersections began providing services to people with intellectual and developmental disabilities in 1995 as an alternative to traditional service models. The company’s mission, To Bring People And Opportunities Together, is accomplished by listening to what people want and then developing services around the individual, rather than attempting to make the person fit the program model. We advocate for individuals with disabilities and promote individual choice, community integration, individualized planning, respect, quality of life and encouraging and developing natural supports. Services are provided in a variety of arenas, including residential, employment, employment training, community participation and transportation. Community Intersections specializes in working with people with intellectual and developmental disabilities who exhibit behavioral challenges and are termed “difficult” by the service delivery system. Through the use of individualized services, the people served have improved their quality of life and found success in participating and becoming an active member of their community.

Connecting with Community, Inc. (CWC) has been providing services through the Supported Living Services waiver (SLS) since 1995. The philosophy of CWC is to assist individuals in achieving greater independence. We believe that this is best accomplished by allowing the individual to have as much control over their life’s decisions as possible. We do this by engaging the individual in defining the challenges they face. We design activities that promote their capabilities. And we focus on connecting individuals, not fixing them. The services that we offer under this waiver include, Personal Care (PC), Homemaker Enhanced (HE), Mentorship, Supported Community Connections (SCC), and hourly respite. All services, except for the SCC services, are provided 1:1. SCC is completed in small groups with one provider that goes out in the community.
For larger groups (more than 3) we use “The Buddy Bus” for our community based activities. These activities include volunteering, bowling/pool, YMCA, hiking/walking/field trips and dances or concerts on Friday nights. CWC also offers an innovative day program, “Club of Arts Learning Center”. Our mission there is to provide an art atmosphere that promotes independence building, creativity, dignity and community integration, by ensuring those with disAbilities have access to artistic expression.
