Ordering Drugs

We’re standardizing our process for ordering drugs. There are multiple ways to do it:

  1. Order drugs through the Henry ScheinMedical (human) in the punch out system.
  2. Set up a Henry Schein Animal Health account. You will be able to order a good portion of your drugs from:
  3. Send the completed applications (attached) to:

  4. include:
  5. Copy of DEA license if you’re ordering controlled drugs (i.e., ketamine):
  6. Go to this website and complete a DEA Form 225:
  7. Any tax exemption information
  8. Put n/a in any field that doesn’t pertain to you.
  9. Request a quote from Clay Barber in order set-up PO. (The email with list of items and prices should be suffice and could be used as quote)
  10. Set up a PO for Henry Schein Animal Health (include quote/email).
  11. For extended release buprenorphine:
  12. Ensure you have a DEA license. They will need a copy of it.
  13. Set up an account from ZooPharm.

  15. Request a check request from Purchasing.
  16. Request a script from Dr. Boehm for the buprenorphine.
  17. Fax it to ZooPharm

  18. The product will be sent after the check is received by ZooPharm.
  19. Order non-pharmaceutical grade drugs as a last resort from Sigma Aldrich or another chemical supplier in the punch-out system.
  20. Make sure you address the use of a non-pharmaceutical grade compound in your IACUC protocol.
  21. Note that this is not allowed for anesthetics or analgesics.
  22. Abide by the IACUC policy #21. If you would like additional guidance, please speak with Dr. Boehm.
  23. Complete the LARC order form above. LARC will return any form that is incomplete. Drugs will be charged back to the investigator, and drugs that take longer than 1 hour to source will have vet or technician time charged back to the investigator, per the new guidelines sent out with the new per diem schedule.

I hope this is helpful. This process will most likely evolve, however we’ve had so many requests that we wanted to distribute the information we currently have. We intend to put it up on the LARC website once it is complete.
