NOTIFICATION OF PRIMARY TEACHER APPOINTMENT FORM (PTAF1)
Dear Chairperson/Principal,
Please find enclosed one Notification of Primary Teacher Appointment Form (PTAF1) which replaces Notification of Permanent Appointment and Notification of Temporary Appointment forms. All permanent, fixed term (temporary) and Contract of Indefinite Duration appointments should be notified to this Department using this new form.
Additional copies of the Appointment form can be downloaded from the Departments website www.education.ie under Education Personnel/Primary/Circulars & Information Booklets – Appointment/Redeployment. The form enclosed can also be photocopied for use.
Please note that when printing or copying the appointment forms the PRSI mandate form (part 5 of the form) must also be printed or copied, signed by the teacher and attached to the appointment form. You should compare the appointment form you download from the website with the one enclosed to ensure that you have printed the entire form.
