Customer Consent Form – One Woman Can Beauty Contest
Dear Customer,
Thank you for agreeing to enter the Mary Kay One Woman CanTM 50th Anniversary Beauty Contest. Mary Kay Philippines, Inc. will be managing the Beauty Contest using internet based tools. To participate in the contest you will need to consent to certain information being used for that purpose.
In particular, as your Independent Beauty Consultant I will take two photos of you: one before your makeover and a second after your makeover. I will upload these photos into the contest website along with my contact information and your active email address. Before this can become an official entry, the website will send an email to you to confirm that you would like to participate in the contest and to allow you to complete the entry form. After you complete the entry form and confirm that you want to enter, your entry will be complete!
After you confirm your entry, your photos will be posted along with your response, first name, last initial, and country for a public voting stage so that your friends and others can vote for you to be a finalist. There will be a Global Gallery of entrants to promote the contest and each market will have their own public voting site, but all of the public voting (including photos, response, first name, last initial, and country for each participant) will be viewable worldwide.

Because your privacy is important to me and to Mary Kay, I want to make sure you are aware that you have a choice with regard to what information you choose to share with me and what information I may share about you with Mary Kay. If you wish to enter the Beauty Contest then please indicate your consent to allow me to share your photos and email address with Mary Kay. Mary Kay (and its affiliates) will only use the information you provide for the purpose of conducting the contest as described in the Official Rules and will not sell or otherwise disclose your information to any third party. Your data will be transferred by Mary Kayto be processed by its vendor W.illi.am on a secure cloud computing platform provided by Amazon servers in Montreal, Canada. Your data will be used only as described in the Official Rules and in compliance with the Mary Kay Global Privacy Policy, W.illi.am Privacy Policy, and the Amazon Privacy Policy. Your data will be kept until December 31, 2013, at which time it will be deleted.
