Job Title: Full Time Social Services Provider I Position number: 45814

Description of Duties:

Under general supervision, provides evaluation and therapeutic services to adult, adolescent and children mental health consumers in an outpatient clinic. Responsibilities include completion of complex psychosocial assessments, treatment team participation, treatment planning/monitoring and discharge planning. Provides individual, family and group counseling to consumers and their families.

Minimum Qualifications:

LCSW, LMSW, LPC, LAPC, LMFT or LAMFT OR Master’s degree in social work, psychology, or counseling which must include a supervised clinical practicum and must be registered with the Georgia Composite Board of LPCs, LCSWs and LMFTs to seek associate license, OR must be completing a supervised clinical practicum as a second year student in a master’s program of social work, psychology, or counseling in addition to having completed a Bachelor’s degree in social work, psychology or counseling and have 2 years full time or equivalent part-time experience as a provider in a social services field. A criminal records background check and pre-employment drug screen is required. If appointed, applicant will be subjected to a Fingerprint Based Background Check of the Georgia and Federal Criminal History Record Databases.

Preferred Qualifications:

Applicants with an LCSW, LPC, LMFT with three or more years work experience in an MH/DD/AD office performing therapist/case management duties.
