About Medical Services and Supports for Persons with Disabilities（for mental clinic）

Medical Services and Supports for Persons with Disabilities are one of the public medical fund’s financial assistance systems which were founded ofAct on Support for Self-reliant Living of Persons with Disabilities.

【Self pay burden】

Essentially, a self pay burden needs a patient to pay 10% with whole medical expenses, but a payment for the burden will be set on a limit on each month as an income etc. may change on each month. Also, the pay burden may become ineligible with depended on the grand total of the municipal inhabitant's tax （per income levy）.

☆ The municipal inhabitant’s tax can be defined as an income 2 years ago if applied until June or it can be defined as an income last year if applied after July.

☆“Mental Services and Supports for Persons with Disabilities Treatment Care Pass”(for mental hospitals) can useat only those described medical institutions and pharmacies.

When consulted the doctor, “Mental Services and Supports for Persons with DisabilitiesTreatmentCarePass”(for mental hospitals) needs to be submitted, and the amount of self pay burden must be described as having a limit administration table.

☆The system can not be used with an accident and sickness(cold or injuries) which are not mental illness related.

☆ The valid period is 1 year. If extension necessary, re-authorization procedures can be started 3 months in advance with the expiry. Please start the process as early as possible as whole procedures may take about 3 months to be done.

☆If the medical institution or pharmacy on the Pass needs to be changed, its change has to be declared the first, so it can start to use after authorized.

・An Application Form with Medical Services and Supports for Persons with Disabilities-supply（The application will be filled out at the reception in city hall.）

・A Doctor’s Certificate for Medical Services and Supports for Persons with Disabilities(issued for mental hospital).

・A Certificate of Taxation with Municipal Inhabitant’s Tax in Previous Year

（If a patient has resided in the city by January of the applying year, the patient does not need the certificate since the patient has written a letter of consent for a study of the taxation condition at the application filled out. However, a patient who has moved from outside the city needs the certificate.）Notice of Decision etc (For those involved many on medical care expenses at high costs.) As depended on an income classification, (Low income 1, Low income 2) the applicant may submit the proof of the pension for handicapped, notice of bank transfer etc to prove the amount of pension that you saved.
