Logic Model Sample

Goal Addressed / Objective / Measure (including time interval) / Participants / Tools/Resources Required
Short-Term Outcomes (~2 months; Changes as a direct result of the strategies/ activities, relative to the contributing factor)
Improve retail establishment employees ability to check and identify fake ID’s(From “contributing factor “column) / By (date), young adults will report that they are less likely to obtain alcohol through fake IDS by X%, from ___ to ___, as measured by MYSA (indicate years) / MYSA / Young Adults 18-20 / ___will collect, store, process, analyze and disseminate the information.
Intermediate Outcomes (~12 months; Broader changes related to the intervening variables)
Young Adults are not able to use fake ID’s to access alcohol in retail establishments (From “Intervening Variables” column) / Within X years of program implementation, there will be a decrease in the retail availability of alcohol for young adults by X amount, from __ to __, as indicated by MYSA (indicate years) / MYSA / Random sample of young adults living in X communities / ___will collect, store, process, analyze and disseminate the information.
Long-Term Outcomes (~24 months; Broadest changes related to the state priority)
Reduced past 30-day use of youth and young adults. (From overarching “State Priority” example) / By the end of MSPF2, youth ages 12-17 who report past 30 day alcohol use in Maryland will be reduced by X%
(from __ to __) , as indicated by YRBS data (indicate years) / State data on alcohol consumption for young adults, obtained from YRBS / Residents of MD / ___will collect, store, process, analyze and disseminate the information.
