Your plan currentlyrecognizes two diabetic test strip products in the preferred brand tier. The preferred diabetic test strip products include ACCU-CHEK® from Roche Diagnostics and One Touch® from LifeScan, a Johnson & Johnson Company.
To have your diabetic test strip products covered at the preferred level, you will need to use one of the preferred systems listed below. If your current diabetic test strips are not preferred, you may be paying the higher non-preferred amount (if your plan is three-tier) or you may have no coverage at all for your current diabetic test strip (if your plan is “closed” or two-tier).
Each of the four glucose systems listed below utilize one of the preferred diabetic test strip. An informational brochure about each is enclosed. To receive one of these glucose monitors at no additional cost, please complete and mail or fax the enclosed glucose monitor form to CIGNA Tel-Drug.
