Enhanced service – Over 40s Men’s Health Check Project

1.  Service description

1.1 This service will enable the pharmacist to measure and assess free of charge the cardiovascular risk factors in men over 40 presenting at the pharmacy, using Joint British Societies’ (JBS) guidelines for the prevention of cardiovascular disease (JBS2).

1.2 Pharmacists will provide evidence based cardiovascular risk assessment including:

·  Assessment of smoking status, alcohol intake, diet, exercise and family history

·  Weight and height measurement to calculate body mass index (BMI)

·  Cholesterol measurement – both total cholesterol (TC) and HDL cholesterol and the ratio (TC:HDL)

·  Blood pressure (BP) measurement

·  Blood glucose measurement and diabetes status.

1.3 The pharmacist will provide support and advice to the patient, including referral to primary care or a specialist service where appropriate.

1.4 This enhanced service is to be provided in addition to the Essential Service ‘Promotion of healthy lifestyles (Public Health)’ (ES4) and ‘Signposting’ (ES5).

2. Aims and intended service outcomes

2.1 To identify, assess and manage the cardiovascular risk factors in men over 40 years of age group, living in North Tyneside.

2.2 To reduce the mortality rate from circulatory diseases in people under 75 to 65 deaths per 100,000 population by 2008.

2.3 To reduce mortality from circulatory diseases in people under 75 and reduce to 15% or less the inequalities gap between Neighbourhood Renewal areas with the highest mortality from circulatory disease and the population of North Tyneside as a whole by 2010.

2.4 To increase life expectancy at birth to 78.6 for men and 82.5 for women by 2010

2.5 To reduce the gap in life expectancy between Neighbourhood Renewal areas and North Tyneside as a whole to less than two years for men and to less than one year for women by 2008.

3. Service outline

3.1 The pharmacy must have a private consultation area which meets the regulatory requirements for privacy.

3.2 The pharmacist must obtain all relevant information from the patient to determine:

·  Smoking status, Alcohol intake, Diet, Exercise and family history

·  Weight and height measurement to calculate body mass index (BMI)

·  Cholesterol measurement

·  Blood Pressure (BP) measurement

·  Blood Glucose measurement

3.3 The pharmacist must calculate the cardiovascular risk of the person using the JBS2 chart

3.4 The pharmacist has a duty to ensure they have completed the appropriate training required to provide the service.

3.5 The accredited pharmacist must ensure that in his/her absence, locum pharmacists are appropriately accredited or know how to refer patients to an alternative service.

3.6 The pharmacist should maintain appropriate records to ensure effective ongoing service delivery and audit.

3.7 The pharmacist will maintain confidentiality of information unless guidance around safeguarding children requires information to be passed to appropriate partner organisations.

3.8 Attendance at biannual update training will be required to maintain accreditation.

3.9 The PCT will provide appropriate paperwork for the recording of relevant service information for the purposes of audit and the claiming of payment.

3.10 The PCT will provide details of relevant referral points which the pharmacist can use to signpost service users who require further assistance.

4. Suggested quality indicators

4.1 The pharmacist can demonstrate that (s)he has undertaken CPD relevant to this service.

4.2 The pharmacy participates in a PCT organised audit of service provision and attends the biannual update training.

4.3 The pharmacist co-operates with any locally agreed PCT-led assessment of service user experience.
