Medication Redesign

The medication ordering redesign is to simplify the medication ordering process. The initial focus is on 32 antibiotics found in the Sepsis Antibiotic Advisor. This Advisor will be available as a stand alone orderable as well as from within a Sepsis PowerPlan. The Sepsis Advisor will be available in mid December.

**Note: Antibiotics that have a Guidance Window are designated in the list above with an asterisk. Silver Spring affected antibiotics are circled in red
Some of these antibiotics include a Physician Authorization field that will be a mandatory field, and a guidance window may display additional information related to the safe and effective use of the antibiotic. The physician may either cancel the selected antibiotic or complete the order from the guidance window. This orderable includes a Physician Authorization field with a pre-populated list of selectable SS Infectious Disease Physicians. You will need to type in ‘SS”, and the Silver Spring MDs will be listed.

Holy Cross P and T Committee has determined that the following drugs need an Infectious Disease Physician Consultation:
  • Amikacin
  • Daptomycin
  • Caspofungin
  • Tigecycline-not in the initial redesign, but requires an ID consult at SS.

Laboratory Orderables Change

The laboratory orderables for Amikacin, Gentamicin, Tobramycin and Vancomycin will be changed as part of a Medication Ordering Redesign in the following ways:
1. The naming convention will be changing on the Peak and Trough orderables.
2. The collection priority will be changed to ‘Timed’ on all orderables (except non-timed Levels which will be routine) and the order sentence will have specific draw instructions
3. Vancomycin Peak will be removed from the catalog and no longer available.
4. Amikacin, Gentamicin, Tobramycin and Vancomycin Random will be hidden from the General Catalog and replaced by Level (use ‘contains’ to search)
PowerPlans and favorites are not impacted with this change. /

HCH MD Resources Website

The Holy Cross Hospital MD Resources Website contains updated information on Meaningful Use, Physician Job Aids for key areas of PowerChart, and Physician Resources such as information on Sepsis and Medication Reconciliation. The Pharmacy Information link keeps you informed of a local drug shortage status, and therapeutic interchange list. The icon (shown on right) is visible on the opening Novell window, the same window where the PowerChart Icon is located. /

PowerPlan Change

The Echocardiogram (SS) PowerPlan will be changed effective Nov 23. Some orderables will be removed to prevent duplicate studies from being ordered.

Frequency Fields on Lab Orders

The Frequency, Duration, and Duration Unit fields will be removed from some Lab orders. The orders that will be affected are: most Body Fluid orders, CSF orders, and Urine Culture orders. If the orders affected have been used in orders sets or power plans, the defaults will remain. Single order sentences will have these field defaults removed. /
