Confidentiality Agreement for Volunteers

Confidentiality means… To keep secret, to not tell!

When you are volunteering in Covenant Health facilities, you will hear and see things that are private and personal. Some examples include:

·  Hearing about peoples’ health problems

·  Seeing people in situations where they are not well and may be having problems

·  Recognizing people you know who are receiving care or treatment

·  Observing staff problems or conflicts

·  Seeing the personal health records of patients or residents

Confidentiality of information is governed by Covenant Health policy and provincial and federal privacy laws.

Your Obligations

Any information you see or hear about patients, residents, or staff must not be shared or talked about unless it is with someone who has the authority to know the information (such as a Covenant Health manager) or when it is required by law that you disclose the information.

You are not allowed to:

x  Make any recording of activities that you hear or observe while volunteering. This means you cannot use any type of electronic device, paper or other format to record information for personal purposes.

x  Post information or your personal comments on any social media site such as Facebook, Twitter, blogs, etc regarding any specific incidents you become aware of while volunteering.

x  Represent yourself in any way as authorized to speak on behalf of Covenant Health.

If you know something that you believe would put someone at risk (if not told), you should inform your supervisor, a manager, or the coordinator of volunteers.

Your signature indicates that you have read the above and agree to follow the rules to ensure confidentiality is maintained.
