Transforming End of Life Care and Experience
Can you help to change and influence the way end of life services are provided to support our local communities?
We would like to offer local relatives and friends who have been recently bereaved an opportunity to share their experiences about what worked well, what didn’t and what else may have helped? The purpose of this is to help us to plan and provide better end of life care based on people’s actual experiences instead of what professionals think is needed. This information is being shared with local end of life care providers, including acute care, community care and hospices as well as influencing the local commissioning processes.
We have planned an event to enable people to share their experiences.
Thurs 23rd April 6:30pm – 8pm.

It will involve participants discussing the things that were important to them in small informal groups. We realise that this is a sad and difficult time for people and there is no need for people to discuss anything that they don’t wish to or stay any longer than they want to. All information shared will be treated respectfully and confidentially and any identifiable details will be removed from final reports.
If people would like to share their experiences but don’t feel that attending an event is right for them, there are a number of other ways in which they can participate:

· A questionnaire is available, which can be either posted or emailed.
· The person can take part in an informal interview, either in person or over the phone.

There is no specific requirement to the relationship of those taking part, but we would ask that their family member or friend passed away in our local area within the last 18 months.
