U9-U10 ACADEMY FEE AGREEMENT

_____1. I understand that all U9-U10 Academy Club Fees are duemonthly toSamba Futbol Club.

_____2. I understand that all U9-U10 Academy Club Fees are due by the 15th of each month.

_____3. I understand that in the event that my payment Samba Futbol Club is made after the 15th of each billing month it will be subject to an additional late payment charge of 25% of the monthly fees due.

_____4. I understand that my child will not be allowed to attend Club functions or activities including games until my fees are made current.

_____5. I understand that monthly Club Fees are required regardless of whether a player participates in the training activities offered.

_____6. I understand that my payment can be made in the form of cash, check or money order or credit card online at Make checks payable to Samba FC.

_____7. I understand that Club fees are used to pay for training, field usage, field lights, and equipment, management fees and materials deemed necessary by Samba FC to further excel my child’s understanding and development of competitive soccer.

_____8. I understand that along with monthly fees due to Samba Futbol Club that I will also be responsible for league, indoor facility and tournament fees as directed by the coach of my child’s coach. These fees will be dividedly evenly amongst all rostered players whether they choose to participate or not. Additional fees will be paid directly to my child’s coach.

_____9. I understand that it is my sole responsibility to provide and purchase for my child a uniform, soccer shoes, shin guards, correct size ball, and water for training/practice and games.

_____10. My child will attend all Samba Futbol Club trainings/practices and games in full uniform and appropriate attire.

REFUND POLICY

  1. Refunds of any kind will not be givento parents/players who choose to leave Samba Futbol Club voluntarily.
  1. Refunds of any kind will not be givento parents/players who are asked to leave Samba Futbol Club due to disciplinary action being taken against them.
  1. Refunds may be given at the discretion of the Samba Futbol Club to parents/players who have faced a financial hardship.
  1. A request for a refund due to a financial hardship must be emailed to
    or review by the Samba FC Board of Directors.
    Parents will be notified by email of approval or non-approval
    within 30 days of receipt of a refund request.
