PERMISSION SLIP: 6th Grade Health - Family Life Unit

Dear Parents/Guardians:

Your child is enrolled in 6th Grade Physical Education, he/she will receive instruction in Family Life/Human Development, Disease Prevention, and Sexual Harassment during Third Term.

During the unit, boys and girls are separated for instruction regarding puberty and human development but receive information on disease prevention and sexual abuse and sexual harassment together. You are invited and encouraged to review all of the materials used, and ask questions you may have during the Parent Preview.

We believe that communication between parents and their children about these important topics is vital. To this end, an opportunity to meet the teacher and preview all materials will be held on:

If you cannot make the meeting, some of the materials may be checked out for viewing from the Professional Library and Instructional Materials Center, located at 44 West Frederick St., Walkersville, MD 21793. Resource information, as well the list of videos that parents can check out to view in advance, can be found by visiting our website: http://education.fcps.org/healthpek12 then by clicking on the Material Reservations tab; choose the appropriate level for your child. For additional copies of the family life permission slip, click on the Permission Forms tab and select the appropriate Family Life permission slip for your child. Those students who do not have permission to participate in the family life unit will be provided alternate assignments and typically will complete the alternate work in a different classroom.
