Read all the “Instructions” below your information before adding your particular course work. Make sure the course “Title” matches the one listed on your transcript. Delete all courses on this sample that you did not, or will not, take.

GAs should remember that courses that do not satisfy degreeor GA requirements will not be covered by the GA fee waiver. ENG 703 should be listed under GA Requirements. Note, too, that ENG 703 does not count toward your credit totals on this sheet. Undergraduate language courses are not covered by GA fee waivers unless a GA receives prior approval from the Graduate College. (If you are a GA and would like GA waiver funding for undergraduate language classes, then contact the Graduate Director.)

In the Semester/Year column, please use the following conventions. To indicate semester, use FA, SP, or SU. To indicate year, use only the last two digits of the year (for example, 13 for 2013). Thus, the Fall semester of 2013 would be FA 13.

As you choose courses, keep in mind that at least 18 credits must be at the 700-level.

Electives should be chosen with your professional and intellectual interests in mind. You should also consider which courses will help you complete the research and comprehensive exam requirements.

Lastly, keep a copy of the Program of Study form for your records. If, after submitting this form, you choose to revise your program plan, then you must file a revised Program of Study form. If you do not, your graduation may be delayed.
