HOW TO MAKE YOUR APPOINTMENT

ON THE DAY ASSESSMENT
Telephone assessment – call reception (826025) if you have a problem needing attention ‘on the day’ or if you feel your medical condition cannot wait until the next available appointment. The duty doctor will call you back and assess your needs over the telephone. The doctor will then arrange treatment or an appointment as appropriate as they have access to appointments that the Reception Team do not.
You must provide a contact telephone number and be available for when the doctor calls you back. It is useful if you can offer some details about your condition so that the duty doctor can prioritise their work load. Please telephone as soon as you can but it can be very busy when the telephone lines open at 8:30am. We do not offer a walk-in assessment service
ROUTINE APPOINTMENTS
·  Internet to book on-line (and also to request repeat prescriptions) please register at Reception for access as photo ID is required
·  Phone Direct line 01932 826025 (open 8:30am - 6:30pm Mon-Fri)

·  In person at Reception
Bookable up to 8 weeks in advance (details of our current appointment times are available from reception).
Please book as you leave the surgery (or as soon as possible) if the doctor or nurse asks to see you again.
We endeavour to book your doctor of choice but if this is not possible within your time frame, we will offer an alternative doctor or that a message is put through to your doctor of choice who will contact you when they are next available.
When you arrive in reception for your appointment remember you can use the self check-in to save you queuing at the desk.
As a courtesy to others please do not use your mobile phone when in the waiting area or during your consultation.
Extended opening hours – routine appointments are available early morning Thursday and Saturday mornings between 9am-12noon for appointments (Reception desk open 9 -11am)
Cancelling your appointment – either in person/by phone/internet/email () ……..please remember to cancel with as much notice as possible (when we are closed a message only can be left on 01932 826008). Last year 1520 patients failed to turn up to their appointment with a nurse or doctor!
Occasionally emergency and complicated consultations can delay your doctor or nurse but it is important that you arrive promptly for your appointment. If you are late in arriving you may be asked to re-book.
TEXT reminders – if we have your current mobile number we will send an SMS text reminder of your appointment 48hrs in advance. Please inform reception staff of your mobile number and remember to advise us if it changes. Please let us know if you no longer wish to receive text reminders.
HOME VISITS Week days for housebound patients or those too ill to attend the Practice, telephone 01932 826023 before 10am if at all possible. The duty doctor will usually call before visiting to discuss your medical problem.
Choosing which out of hours’ service to use is important to ensure you receive the right treatment fast, leaving the emergency services free to deal with those who need them most.
Urgent Medical Problems when we are closed
1.  A well-stocked medicine cabinet will help you treat many illnesses yourself at home.
2.  Look under “NHS Choices” on the internet – this contains helpful advice and guidance.
3.  Local pharmacists are qualified to give advice on illnesses and the medicines you need to treat them.
4.  NHS Walk-In Centre 01932 826013 - located in the same area as the treatment room on the first floor. Patients can be seen by an experienced Nurse Practitioner without an appointment for minor injuries and illnesses - they do not treat ladies who are pregnant or babies under 2 years old. Opening hours are 7am to 7pm week days and 9am-4pm weekends bank holidays. There is also an X ray facility Monday to Friday 9-5
5.  The new NHS telephone service 111 – dial 111 to telephone for confidential health advice and information 24 hours a day i.e. the location of an out of hours emergency dentist or pharmacy. The new 111 service is available 24 hours a day 365 days a year. Calls are free.
6.  The new NHS telephone service 111 will also direct you to the most appropriate health care service such as the GP’s out of hours service (Care UK) - if you have an urgent medical problem when we are closed dial 111. An operator will take details and if appropriate will refer your details to the out of hours duty doctor who will phone you, either with advice, ask you to visit a primary care centre where a duty doctor is located or visit you at home.
7.  Please remember that A & E departments treat accidents and emergencies and should only be used in serious and life-threatening situations (e.g. someone has collapsed or is showing signs of a stroke). Should a patient be experiencing chest pain accompanied by nausea and sweating, dial 999 and inform the Ambulance Service.
Please note that your GP reviews A & E attendances and may write to you with details of more appropriate services you could have used.
If you have any concerns or questions about our appointment system or our services, please inform us via email , complete our comment/suggestion form or speak to a member of staff.
