RE:Advisory Memo - Arkansas Department of Health Food Service Inspections

Effective January 1, 2005, the Arkansas Department of Health (ADH) will begin conducting Food Service Inspections in the above-listed facilities licensed for 11 or more beds. Inspections will cover such areas as Food Protection, Personnel, Sewage, Plumbing and Other Operations. Each facility will receive a copy of the Arkansas Department Food Establishment Assessment Report at the time of the Assessment. Office of Long Term Care inspectors will request a copy of the report to ensure compliance with state laws and regulations.
Applicable to Chapter 11 in the Rules and Regulations Pertaining to Food Service Establishments, Arkansas Department of Health under the Authority of Act 96 of 1913 as amended, you may not operate a food service establishment without a valid food establishment permit. The permit may be obtained by completing an ADH application form and attaching any applicable fees.
Those facilities licensed before January 1, 2005, will not be required to submit blue prints to the Food Service section of the ADH. Facilities under construction or facilities that plan on remodeling after January 1, 2005, shall submit blue print plans to the ADH for approval as well as the Office of Long Term Care (OLTC).
