NotesA School COSHH form in Word is available on the School Server.

Available from the Health and Safety Unit.

Guidance on making an assessment is given in Making a Chemical Hazard and Risk Assessment.

Guidance is also available from Guidance on Completing the Chemical Hazard and Risk Assessment Form.

Use a continuation sheet to expand any section of this form in hard copy version.

1 / LOCATION OF THE WORK ACTIVITY / N114
2 / PERSONS WHO MAY BE AT RISK
List names where possible / R.Perry,M.Stanley,A.Humphries,K.Marshall,J.Finlay,R Holland,M Callow, project students and any other persons working in N114.
3 / ACTIVITY ASSESSED / Staining of algal cells
4 / MATERIALS INVOLVED / Attach copies of data sheet(s)
NAME / AMOUNT
max/stored / HAZARD / RISK PHRASES / HAZDAT NO*** / BIOSCIENCESNO***
Alcian blue 8GX / 25g / Harmful
Toxic / Harmful by ingestion
Toxic by ingestion and in contact with skin
5 / INTENDED USE**
Give brief details and attach protocol/instructions
As 0.01% solution in sea water to be used to stain algal cells
Solid stored at room temperature in Toxic cupboard in laboratory N109
6 / RISKS to HEALTH and SAFETY from INTENDED USE
From personal exposure or hazardous reactions. Refer to OELs, flash points, etc., as appropriate. Are pregnant women, breast-feeding mothers especially at risk?
Main risks from ingestion or skin contact during preparation
Low concentrations used (0.01%), therefore risks reduced when in aqueous solution
7 / CONCLUSIONS ABOUT RISKS
Is level of risk acceptable? Can risk be prevented or reduced by change of substance/procedure? Are control measures necessary?
Risk is minimal so long as good chemical practice is followed and the correct protective clothing is worn such as lab.coat, gloves and eye/face protection.
Amounts used in solution and in procedure are relatively small so hazards should be reduced.
8 / CONTROL MEASURES
Additional to Good Chemical Practice
Wear vinyl, latex or rubber gloves when handling solid or solutions
Wear appropriate eye protection
Use dust mask to avoid ingestion/inhalation of powder
9 / INSTRUCTION/TRAINING
Specify course(s) and/or special arrangements. / None
10 / MONITORING
Performance of control measures,
None
Personal exposure / Health Surveillance
11 / WASTE DISPOSAL PROCEDURE
See School Server for Approved Procedure Document on specific Chemical Waste Disposal.
Amounts used are small and therefore solutions can be diluted with copious amounts of water and disposed of in a sink within a fume cupboard.
12 / REVIEW
Enter the date or circumstances for review of assessment (maximum review interval 5 years)
5 years or sooner if quantities or procedures alter significantly
13 / EMERGENCY ACTION
TO CONTROL HAZARDS / To stabilize situation eg spread absorbant on liquid spill; eliminate sources of ignition, etc.
Wash site of spillage thoroughly with copious amounts of water and detergent
In case of large spillage, contain with sand or earth
TO PROTECT PERSONNEL / Evacuation, protection for personnel involved in clean-up, Special First Aid
Ensure protective clothing is such as lab.coat, gloves and eye/face protection, to ensure risk is minimalised
Eyes: Irrigate thoroughly with water for at least 10 minutes. If symptoms persist, obtain medical attention.
Skin: Wash thoroughly with water. In severe cases, obtain medical attention.
Mouth: Wash out mouth thoroughly with water and give plenty to drink. Obtain medical attention.
Contaminated clothing should be washed before re-use
TO RENDER SITE OF EMERGENCY SAFE / Clean-up/decontamination
Wash site of spillage thoroughly with water and detergent
