Platform Simultaneous Operations Agreement

Between On-Site Consultant and Production Representative

This document provides the On-Site Consultant and the Production Representative with topics for mutual discussion and agreement prior to initiating drilling, construction, workover, P&A work on an Apache operated platform. The On-Site Consultant is defined as the supervisor in charge of on-site activities related to the drilling, completion, re-completion, workover, P&A or construction operations on the platform, usually under the control and direction of the Houston/Lafayette engineering department. The Production Representative is defined as the Person in Charge, Lead Operator, or his designate, responsible for the production activities on the platform. Both parties have a shared, common goal of the highest priority: to ensure that all simultaneous operations on the platform are conducted safely. This Platform Agreement is intended to clarify the roles and responsibilities of each party and should be completed when one or more of the following conditions exist:

1.  The On-Site Consultant arrives on the platform to start work on a new project,

2.  The Production Representative selected to coordinate with the On-Site Consultant on the platform has not previously met with the On-Site Consultant (such as might result from hitch changes) or,

3.  Production or drilling, construction, workover, P&A, completion operations in the field have changed to the extent that the either party deems it appropriate to re-orient the other party.

Topics for Discussion:

Review of Planned Drilling, Workover, P&A and/or Construction Activities to be accomplished on the Platform: The On-Site Consultant shall brief the Production Representative on the general scope of activities he plans to accomplish. The Production Representative will consider the work to be done and inform the On-Site Consultant of any site specific conditions that may impact the work, including, but not limited to: general safety issues, use of the platform crane, any areas to be avoided due to hazardous atmospheres or conditions, and condemned areas. The Apache Simultaneous Operations Procedure shall be reviewed and discussed. See Section D Chapter 26.

Notes on issues discussed:

Recognition of Person in Charge: Apache recognizes that the Production Representative is the Person in Charge of the platform. This responsibility necessitates certain “high level” authority over all activities occurring on the platform, including workover, drilling, P&A and construction operations. The On-Site Consultant is responsible for safety and technical matters relating to operations under their control. It should be made clear however, that the On-Site Consultant shall respect the Production Representative’s decisions regarding general safety on the platform, the general conduct of personnel under the On-Site Consultant’s control, and housekeeping. This in no way is meant to influence, limit or impact the On-Site Consultant’s best judgments regarding safety or technical decisions for the specific work he is accomplishing.

Notes on issues discussed:

Mandatory Notification of Incidents: The On-Site Consultant and Production Representative have a mutual interest in being notified of certain incidents that occur within the other party’s operations. Either party shall notify the other party in the event of:

·  Any injuries, fires, explosions or near miss reports that occur within their respective operations.

·  Any instances where a well is considered “Out of Control”.

·  Any prolonged downtime that might affect either operation including, but not limited to, the ability for the well to be tested or hooked up to the production processing equipment.

·  Any damage suffered to platform equipment such as cranes, process vessels, or safety systems.

·  Any planned inspections from regulatory bodies such as the Coast Guard or BSEE.

·  Any spills or releases of substances (oil, chemicals etc) that are not permitted as part of normal production, workover, drilling, P&A or completion operations.

·  Other:

Note: This in no way impacts the requirement for the On-Site Consultant or the Production Representative to make required notifications within his chain of command (i.e. to Houston Engineering, Production Operations, EH&S etc) for injuries, spills or other incidents as prescribed by Apache GoM Shelf Region Safe Work Practices.

Emergency Evacuation Procedures: The On-Site Consultant and the Production Representative should discuss the emergency evacuation procedures for personnel onsite. Muster areas should be discussed and clearly identified. A copy of the daily Person on Board form should be distributed to both parties each day.

Drills: The On-Site Consultant and Production Representative should make each other aware and should participate in any drills planned within their respective operations.

Safety Meetings and Job Safety Analysis: The On-Site Consultant and Production Representative should routinely participate in safety and job planning meetings in order to keep one another informed of critical job tasks.
