LOOPS COMMUNITY WELLBEING TEAM
·  LN said the LOOPs Hospital Discharge Team is now being called the LOOPs Community Wellbeing Team.
·  Said the team’s Hospital Liaison Workers are undertaking development work at the moment.
·  The programme is aiming to begin delivery in either the first or second week of September.
·  Details of the programme’s launch events can be found in the flyer attached in Appendix A.
LOOPS PHONELINE
·  HY updated attendees on the calls coming into the LOOPs Phoneline. A list of queries can be found attached in Appendix B.

SOUTH WEST COMMUNITY NAVIGATOR PROGRAMME
·  In Edyta Pyczek’s absence, DB updated attendees on the progress of the Community Navigator work going on in SW Edinburgh.
·  Slides of this presentation can be found attached in Appendix C.
·  The Community Navigator programme is interested in reaching less mobile older people who are less likely to be able to access one of the programme’s Information Stalls out in the community. It is also interested in finding more dates and venues for its information stalls. To help with either of these, please contact Edyta Pyczek, Community Navigator, at:
VOOPs
·  DB updated attendees on progress with the VOOPs survey. Slides attached in Appendix D.
·  The outputs of the survey are currently being compiled into a report that will be sent to commissioners – City of Edinburgh Council, NHS Lothian and Edinburgh Health and Social Care Partnership. This report will include recommendations for the best ways to incorporate the views of older people into the services that affect them.
·  Commissioners are expected to respond to the recommendations in the report by the end of September.
·  A launch event for the report will take place on 13 October. Details to be publicised nearer the day.
ANY OTHER BUSINESS
·  RH updated the attendees on the Care Home Olympics.
o  On 26 & 27 July, more than 90 residents of the council’s 11 care homes took part in the inaugural Care Home Olympics. Events included javelin, new age Kurling, Unstrictly Dancing, Care Home Does Count Down and Funky Moves courtesy of Edinburgh Leisure.
o  RH will be compiling a resource pack based on the day for other organisations interested in hosting a similar event.
·  HY reminded people that Volunteer Edinburgh’s Volunteer Recruitment Fair will take place on Wednesday, 21 September 2016 between 11am and 7pm at St Paul’s and St George’s Church on York Place.

·  AS informed attendees that Home Instead will be hosting a MacMillan Coffee Morning event on Friday, 30 September, 10:30am-1:30pm.

Appendix A:

LOOPs Community Wellbeing Team

Locality Launch Events

Within each of the new Health and Social Care Localities, the Edinburgh Health and Social Care Partnership (also known as the Integrated Joint Board or IJB) is developing ‘Locality Hubs’ which will bring together community staff from all sectors and a wide range of disciplines. The key focus for the new locality hubs will be delayed discharges for older people and finding new ways to ensure that older people remain in hospital for the shortest period possible and to strengthen the support within the community on their return.

As part of this development, EVOC, The Eric Liddell Centre, Health in Mind and Libertus Services have a team of staff placed within the new locality hubs.

To find out more, you are please come along to the one of our local events. These events are a chance for us to tell you about our role within the hubs and how you can become involved.
