Family Support Program Outcome Survey
Program Codes
Please use this model when assigning program codes for your courses. This uniform method of coding will ensure consistency of data collection and data reporting.
A program code can be broken down into four parts: category, location, class name, and group/evaluation number. For example:
PEFARBB001
1 2 3 4
1)The PE represents the category. (ex. Parent education)
2)The FAR represents the location (ex. Fargo)
3)The BB represents the class name (ex. Bright Beginnings)
4)The 001 represents the group of evaluation number for that specific category, location, and class.

4)Group or Evaluation Number (PEFARBB001)
This number represents the group or evaluation number for each unique category, location, and class combination. This number is three digits, so add leading zeros when needed.
This may be beneficial if you want to run a report on each specific group. For instance, you would like a report on the Bright Beginnings class held at one location (PEFARBB001) as well as a specific report for a second location (PEFARBB002). However, if you wish to run reports on the entire Bright Beginnings training for the entire year, then it would be more useful to use the same evaluation or group number (i.e. use PEFARBB001 for all).
