Ref: Request for Quote forProvision of Complementary Therapiesat Families Beyond Conflict, 56Tennent Street, Belfast BT13 3GB

The purpose of this letter is to solicit a quote from your organisation with respect to the following services to be provided at our centre and funded by the Victims & Survivors Serviceunder the Victims Support Programme for Victims and Survivors of the Northern Ireland Troubles:

Our aim is to provide Victims & Survivors in the Greater Shankill area of Belfast with a range of complementary therapies to address the needs of our clients experiencing physical and mental issues due to trauma.

Our requests are as follows:

Services Required
Acupuncture, Aromatherapy, Reflexology, Reiki, Massage - Indian Head, Massage - Holistic

The provision of 2 fully trained and qualified Therapists to supply 6 x 1 hour Therapy sessions per clientin our Therapy Room at our Centre at the above address: We will have a total of 40 clients for year starting April 2013 until March 2014 with a break-down of sessions as follows:

Beginning with 10 clients for 6 weeks during the period of May to June 2013 on Tuesday mornings and afternoons and Friday mornings and afternoon.

Followed by 10 clients for 6 weeks during the period of July to September 2013on Tuesday mornings and afternoons and Friday mornings and afternoons.

Followed by 10 clients for 6 weeks during the period of October to December 2013on Tuesday mornings and afternoons and Friday mornings and afternoons.

Followed by 10 clients for 6 weeks during the period of January to March 2014 on a Tuesdaymornings and afternoons and Friday mornings and afternoons.

If your quote is successful we will require copies of proof of your insurance, proof of therapists C.V.’s, insurance, qualifications and experience to be kept on file at our centre. There will also be a Service Level Agreement for the services you will provide, signed by both yourself and Families Beyond Conflict.

Please reply with a quote if you are:

(a) Able to provide the Services required by Families Beyond Conflict;

(b) Commit to working in collaboration with Families Beyond Conflict over the term of any agreed contractual period to continuously seek improvements in value, efficiency and productivity in connection with the provision of the Services;

(c) Prepared to work with Families Beyond Conflict to continue to identify opportunities for improvement in the quality and level of service provided to us, for the mutual benefit of both Families Beyond Conflict and you the provider.
