DHR Reporting Process Reference Sheet
All employeesare mandatory reportersof suspected child abuse/neglectincluding all certified administrators, teachers and all non-certified staff. This means that whoever suspects child abuse has the responsibility to contact DHR. It is not the responsibility of the School Counselor or School Nurse to make the referral to DHR unless they are directly involved. A referral to DHR requires both a verbal and follow-up written report.

Verbal DHR Report

(The two intake workers at DHR are Mrs. Anna Barrett and Mr. David Copeland. Mrs. Barrett’s telephone number is 947-8354 and Mr. Copeland’s telephone number is 947-8346. )
When Making a Verbal Report:
- Ask for the Intake Department.
- Obtain the following information from the child or INOW and be prepared to give it to the Intake Worker:
- Student’s full name
- Sex
- Race
- Date of birth
- Age
- Address
- Parents’ names
- Telephone number(s)
- Name(s) of sibling(s)
- Ask for the name of the DHR Intake Worker who you are speaking with and write it down. You will need this name for the written DHR report.
- Should you get voice mail, leave a message with your name, title, school, and call back number, the student’s full name, grade level, date of birth, legal guardian’s name, address, and phone number and a brief message as to why you are calling (“ I suspect child abuse because ______”)
- In an emergency/crucial situation, if DHR is unable to respond immediately, or by the end of the school day, call the Sheriff’s Department at 937-0202. (The office hours for DHR are Monday-Friday from 8:00 a.m.-4:30 p.m.) Notify the Principal if you are contacting the Sheriff’s Department.
Written DHR Report
- As soon after the oral report is made, the person contacting DHR (or the Sheriff’s Office) should provide a written report to either DHR or the Sheriff’s Office. The written report may be e-mailed to either Mrs. Barrett at r to Mr. David Copeland at
- Should a report need to be faxed, it will be routed directly to the Intake Department. The fax number for DHR is 945-2491.
- The Principal should always be notified of such allegations pertaining to abuse and/or neglect, and given a copy of the written report.
If you mail your written DHR report, please make a copy for your file. This is your documentation that you made the referral. Record the date that you mailed it on the copy and initial it.
