Health and Benefits Insurance Broker

Proposal Due Date: April 20, 2018

Purpose:

The Arizona Sports Foundation is seeking bids for a health and benefits insurance broker. The scope of this proposal will include medical, dental, vision, life, long term disability insurance as well as health savings accounts and a flexible spending plan. Selected firms may be asked to meet to discuss in greater detail their plans and suggestions for The Arizona Sports Foundation.

General Company Information

The Arizona Sports Foundation is a non-profit entity operating in Scottsdale, Arizona with a current staff consisting of thirty full time benefits eligible staff members and three part-time non eligible staff members. Employees are eligible for benefits on the first of the month following the completion of one full month of employment. Open enrollment is held in the month of March with an effective date of April 1st of each year. Medical coverage is currently provided through Cigna; Dental and Vision care through Delta Dental; Life, AD&D and long term disability benefit coverage is held through Lincoln Financial Group; Health Savings Accounts are administered by Optum Bank and the Section 125 plan is administered by Golfleaf Partners. At present the employees are required to pay a portion of the monthly premium for medical, dental and vision benefits, life and long term disability are paid for by the employer. In addition to these benefits, the Company offers a retirement plan that is not included within the scope of work at this time.

Scope of Service

The responsibilities of the selected insurance brokerage will include but may not be limited to the following:

  • Annually assessing the medical, dental, vision, life and disability and flexible spending plan needs of the company’s benefits eligible staff, researching the benefits market for vendor plans that fulfill those needs and to present findings to the company’s selection committee for review
  • Facilitating open enrollment, new hire orientation, communicate benefit updates and interact as necessary on behalf of the Company and its staff with respect to issues that arise with benefit vendors
  • Act as a liaison on behalf of the Company with selected benefits carriers and their customer service centers, accounting departments and administration team
  • Work with the Company to ensure information on Federal and State levels pertaining to benefits is communicated with the Company and its staff and assist when necessary with Federal and State requirements in regards to compliance and filings to ensure the Company is in good standing at all times

Timeline:

To be considered, submit your responses to the request for proposal along with three (3) client references no later than April 20, 2018. Questions regarding the request for proposal should be emailed to the project manager listed below.

The Arizona Sports Foundation reserves the right to conduct discussions with other vendors. The Arizona Sports Foundation reserves the right to revise and negotiate the proposal. The proposal determined to be the most suitable to the organization shall be chosen based on the evaluation of the factors requested in this proposal. Finalists in this process may be requested to appear before the RFP committee and present their proposal. There will be no obligation between the parties until a valid contract is executed. Selected vendor may be, with notification, subject to reference and background checks.
