Activity Consent Form – 2017 China Study Tour

From 14th September to 28th September 2017, we will be conducting 2017 China Study Tour as part of our Global School Program.

The aim of the activity are to meet the increased demands from the students, parents and community to enhance students’ learning through international study tour; to connect students’ learning wit hthe real world; to build on their entrepraunial skills with global connectedness.

Activity details:

The following details relate to an educational excursion to Mainland China and Hong Kong which is being organised for Cairns SDE full time enrolment students and school based Chinese language students (Year 9 to Year 12 in 2017 and must be born in or before 2002) to experience authentic Chinese culture, and to broaden their global views.

This Study Tour is sponsored by Tourism Confucius Institute at Griffith University, who are responsible for in country activities, transportation and accommodation. Cairns SDE teachers will look after the international travel, Hong Kong visit and bookings. For draft travel itinerary, please find the attached information.

Activity Costs:

The cost of this trip is AU$2000, which includes return air tickets from Cairns to China; transfer airport to hotel; two-night accommodation in Hong Kong; 1 day Disneyland ticket; Chinese visa cost; in China food; activities; transport and tour cost; and $200 of pocket money to cover meals and incidentals which are not included.

This cost does not include travel and medical insurance, vaccinations, some meals on road, transport from home to Cairns nor other personal spending.

A $500 non-refundable deposit is payable to Cairns SDE in Week 4 Term 4 2016. This is guarantee money we need to pay to the travel agency, Escape Travel Cairns, to secure our flight and accommodation booking. For a legitimate

reason that a student could not carry out the travelling, it is up to students’ guardian to negotiate with the Escape Travel Cairns for a possible refund.

16 August 2017 / Passport original copy, 2 colour passport photo, visa application form, medial and travel insurance form, immunisation form, Code of Conduct form back to school for visa application
28 August 2017 / Parents information night – What to pack

There will be three supervising teachers accompanying students for this trip. The cost of supervising teachers are covered by Cairns SDE and Confucius Institute. The contact details of these three supervising teachers are:

Staff accompanying students on overseas excursions will take all reasonable care while the students are in their charge to protect them from injury and to control and supervise their behaviour and activities.

The Department of Education and Training (DET) is collecting the personal information requested in this form in order to:

- obtain lawful consent for your child to participate in the activity;

- help coordinate the activity;

- respond to any injury or medical condition that may arise during, or as a result of the activity; and

- update school records where necessary.

The information will only be accessed by authorised school staff and will be dealt with in accordance with the confidentiality requirements of s.426 of the Education (General Provisions) Act 2006 (Qld) and the Information Privacy Act 2009 (Qld).

Activity Risks & Insurance

Please note that the Department of Education and Training does not have personal accident insurance cover for students. If your child is injured as a result of an accident or incident while participating in the activity, all costs associated with the injury, including medical costs are the responsibility of the parent/carer. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may be also be covered by your provider. Any other costs must be covered by parents/carers. It is up to all parents/carers to decide what types and what level of private insurance they wish to arrange to cover their child. Please take this into consideration in deciding whether or not to allow your child to participate in this activity.

Consent

By signing this form (below) I agree that:

·  I have read all of the information contained in this form in relation to the activity (including any attached material) and I am aware that the Department of Education and Training does not have personal accident insurance cover for students.

·  I will pay to the school the costs detailed above for my child’s participation in the activity.

·  In the event of an accident or illness, school staff may obtain or administer any medical assistance or treatment my child may reasonably require, including contacting my child’s doctor.

·  I accept liability for all reasonable costs incurred by the Department of Education and Training in obtaining such medical assistance or treatment (including any transportation costs) and undertake to reimburse the Department of Education and Training the full amount of those costs.

·  I have provided the school all relevant details of my child’s medical or physical needs on enrolment and where relevant have updated this information.
