☐I intend to continue as a Substitute with Higley Unified School District for the 2017-2018school year. I acknowledge HUSD Substitutes are employed on an at-will basis and certain actions can result in the deactivation of my account in Aesop, the exclusion from individual schools, or termination of employment without notice. I acknowledge that if I have not worked a minimum of 5 times within a six month period my Aesop account will be deactivated and employment terminated. I acknowledge that cancelling from assignments more than two times a month is considered excessive and can lead to my termination as a Substitute for the district.
☐I intend to continue as a Substitute Aide with Higley Unified School District for the 2017-2018 school year. Please be advised, if you would like to continue as a Substitute Teacher and a Substitute Aide please check both boxes.
☐I do NOT intend to continue as a Substitute Teacher and/or Aide with Higley Unified School District for the 2017-2018 school years.I understand my personnel file will remain current for 1(one) year and if I have not worked for the district in that time frame I will have to reapply, at my own expense, before being considered for rehire.
