Field Trip Guidelines

2015-2016

Overnight Field Trip Requestsrequire school board approvalone (1) month out from the date of the meeting. The overnight packet must be submitted to the Associate Superintendent’s office to the attention of Leslie Jacobs at least two weeks prior to the date of the meeting. Overnight field trip requests must be typeand completewith all requested information. Incomplete packets will be returned for completion which may interfere with the two week timeframe for processing for board approval.

Required Information: The name, address, and phone number of the hotel/overnight accommodations, rules for the trip, itinerary, andmode of transportation(bus request/private passenger form). All trips require a 1:10 ratio for chaperones. The board recommends a male and female chaperone to attend overnight field trips if both male and female students are attending.

The board will not approve any overnight out-of-state travel for primary grades (K – 5).

Note:There will be exceptions for “Special”situations that do not meet the two week timeframe for board approval.

Out of State/Day Field Trip Requestswill be submitted to the Associate Superintendent’s Office to the attention of Leslie Jacobs for Dr. King’s approval. Complete packets must be received three weeks prior to the date of the trip. Please include all necessary forms (bus request/private passenger). Incomplete paperwork will be returned for completion and may interfere with the three week timeframe for approval.

Note: Field Trips require a 1:10 ratio for chaperones.

Note: In State/Day field trip request (and Charlotte, NC) will be approved at the school level by the Principal.

Field trip/bus requests must be signed by the requestor and principal before submitting for approval. Private passenger forms must be signed by the owner of the vehicle or the person signing the rental agreement. Non-Preferred Travel Dates:

December 14 - 18, 2015

Note: State testing dates will be non-preferred starting one week prior to testing and ending on the last make-up date.

Note: Non-preferred travel dates must be submitted to the Associate Superintendent’s office for approval

Cut-off Travel Date: May 18, 2016
