·  Complete the attached application (ink or typed) and submit it to the guidance office no later than April 2, 2012.

·  Any additional pages (maximum of 4 total pages) must be single-sided with the applicant’s name on each page.

·  Do not submit your application in a report cover. Affix all pages with a paperclip (p. 2 on top) so they can easily be copied.

·  Failure to complete ALL sections of this application accurately may disqualify you from scholarship consideration.

·  The essay questions (p. 3) are required for scholarship consideration.

·  Retain pages 1 and 3 for your records.

This application will be used to award a number of local scholarships. We are looking for responses that make you stand out amongst the other applicants, and reflect your worthiness as a scholarship winner. Your responses to the following essay questions are one of the most important parts of your application.
