Job Summary
To provide practical, emotional and physical support to people living in the community.
MAIN DUTIES & RESPONSIBILITIES
Direct Support Work with Service Users
- To provide direct physical support and assistance to individuals with a wide range of needs.
- To work unsupervised in a variety of settings.
- To support individuals to access opportunities in training, leisure, education and employment.
- To provide opportunities for individuals to learn new skills and to have a wide range of life experiences.
- To support individuals to create and maintain a living environment which is safe, clean and meets good health and safety standards.
- To assist individuals to make decisions about their lives.
- To provide appropriate and responsible advice toindividuals in dealing with daily living.
- To be a key-worker and support the individual to realise their Person Centered Plan and Health Action Plan.
- To supportindividuals to foster relationships with local people and maintain existing family relationships.
- To support individuals to develop daily living skills (for example day-to-day social and domestic activities, such as cooking, health eating, budgeting and cleaning).
- To support individuals to report repairs to the relevant Housing Association or Local Authority and in some cases arrange access for contractors. In addition to undertaking minor maintenance such as changing light bulbs.
- To support people with their personal appearance and care.
- To work as a team with Outreach Co-ordinators to provide services to people with more complex care packages.
Record Keeping and Procedures
- To be familiar with and act upon all the policies and procedures of theCamdenSociety.
- To use Office Outlook in order to communicate with other members of the team.
- To contribute towards, sign and work in accordance with all Society working practices and risk assessments as well as agreed external assessments written by other agencies.
- To be directly responsible for ensuring that service records are accurately maintained in a professional and confidential manner. Some examples are:
Individual client files Drugs administration records
Communication DiariesIndividual Service Plans
Accident and incident ReportsDaily Records
ADDITIONAL DUTIES AND RESPONSIBILITIES
- To implement the Society’s Equal Opportunities Policy in all areas of work.
- Preparing for, attend and participating in regular team meetings and one-to-one supervision sessions with your line manager as well as attending appropriate service user related meetings as directed by the Deputy Managers and Outreach Service Manager.
- As part of your shift rota, provide sleep over support where required.
- To keep informed and up to date with regards to current initiatives in learning, physical disability and related mental health fields. In addition, to develop a working knowledge of welfare benefits.
- To familiarise yourself with the systems and structures of statutory and voluntary bodies.
- To represent the Camden Society in a professional manner at all times.
- To participate fully in training, as directed by the Deputy Manager or Service Manager.
- Any other duties appropriate to the post of Support Worker, as directed by the Deputy Manager or Service Manager.
