GENERAL REQUIREMENTS FOR ISSUANCE OF PHILIPPINE PASSPORT

*** INCOMPLETE APPLICATIONS WILL CAUSE DELAY IN THE ISSUANCE OF YOUR PASSPORT ***

Only the following States are within the jurisdiction of the Consulate General in Chicago : Arkansas, Illinois, Indiana, Iowa, Kansas, Louisiana, Michigan, Minnesota, Mississippi, Missouri, North Dakota, Nebraska, South Dakota, Ohio, Oklahoma and Wisconsin

A.RENEWAL OF PASSPORT

1.Accomplish application form. Parents and/or legal guardians may accomplish this form for minors.

2.Submit two (2) passport pictures, 1.77” x 1.37” (face length from chin to crown of the head should be between 1.00” to 1.18”), front view, colored or black and white with plain white background taken within the last six (6) months. Photo must show applicant in proper attire without eyeglasses. Casual attire (e.g. sleeveless dresses, collarless t-shirts) are not allowed. Scanned pictures are not accepted.

  1. Submit applicant’s old passport for cancellation
  2. Submit photocopies (xerox copies) of the following documents:
  1. passport pages 1-2 and page with visa and latest arrival stamp in the United States
  2. alien registration card (green/pink card) for U.S. permanent residents
  3. Birth Certificate or any other document showing complete/spelled out middle name

d. proof of actual residence e.g. driver’s license, state ID, employment ID, or utility bills, etc.

5.For minors, submit a notarized Affidavit of Support and Consent from both parents. Please indicate if minor is traveling with both parents. Clearance should be obtained from the Department of Social Welfare and Development if minor child/children is/are unaccompanied by both parents. (For further information, please call the Consulate)

B.REPLACEMENT OF LOST PASSPORT

THERE IS A MINIMUM WAITING PERIOD OF FOUR (4) WEEKS FROM DATE OF APPLICATION.

6.Accomplish Requirements Nos. 1, 2 and 4

7.Submit original /certified true copy of Birth Certificate duly authenticated by the National Statistics Office (NSO), Malacañang, and the Department of Foreign Affairs (DFA) in Manila

If lost or not available, submit the following:

a) Certificate of Non-Availability of Birth Record from the Local Civil Registrar.

b) Baptismal Certificate/Voter’s ID or any public document indicating birth particulars and Philippine citizenship.

c) Joint Affidavit by two disinterested persons.

8.Submit other supporting documents which will establish applicant’s identity as Filipino citizen, e.g. marriage certificate,

school records, professional ID, etc.

9.Submit an Affidavit of Lost Passport, duly notarized or signed before a Consular Officer stating the circumstances

surrounding the loss of the passport. Indicate the passport number, date and place of issue. Attach Police Report

if passport was stolen.

10.Submit photocopy (xerox copy) of lost passport or other previously issued passport, if available.

C.OTHER REQUIREMENTS

11.For female applicant married in the Philippines and who will use her husband's family name in passport for the first time, submit original/certified true copy and photocopy of registered marriage certificate.

12.For female applicant married in the U.S. and who requires amendment of her last name, a Report of Marriage shall be filed in triplicate, together with the following:

a)Original/certified true copy and 3 photocopies of registeredmarriage certificate

b)Photocopy of Birth Certificate or any other document showing maiden name of applicant.

c)One (1) photo each of applicant and her spouse.

d)For widowed applicants - certified true copy of the deceased husband's Death Certificate

e)For annulled marriages - certified true copy of annotated Marriage Contract or Certificate of Registration and certified true copy of Court Order

f)For divorced applicants - original/certified true copy and photocopy of divorce decree.

NOTE : Amendment maybe allowed if the person who initiated the divorce proceedings is the non-Filipino husband. However, amendment is not allowed if both parties are Filipinos at the time of divorce and/or the initiating party is a Filipino citizen.

  1. For retired/resigned Philippine government officials/employees, submit departamental clearance, acceptance of resignation, retirement documents and travel order.
  1. FIRST TIME APPLICANTS

14.For EVACUEES/SERVICEMEN/MILITARY PERSONNEL who came to the U.S. using only military ID and without Philippine passport, submit Req. 1, 2, 7 and 8. A certification from the Commanding Officer to the effect that applicant is not a U.S. citizen who is an evacuee from the Philippines/ on active duty with the US military service is also required.

15.For MINORS born in the US of Filipino parent(s), submit Req. 1, 2 and 5. A Report of Birth shall be filed in triplicate,

by either parent, together with the following:

a)Original and three (3) photocopies of Birth Certificate of applicant

b)Three (3) photocopies of Marriage Certificate of parents

c)Three (3) photocopies of Philippine passport of either parent.

d)Three (3) photocopies of U.S. Passport of applicant, if any

e)Accomplish requirement no. 5

NOTE : NO PERSONAL CHECKS ARE ACCEPTED. Fees are non-refundable.

F.APPLICATION BY MAIL

16.Passport services by mail should be notarized. Applicants has to send the corresponding passport fee in money order or bank draft payable to the Philippine Consulate General. If applicant wishes the passport to be mailed by the Consulate, applicant should enclose a self-addressed prepaid US Postal Service Express Mail envelope or a prepaid DHL/FEDEX/UPS envelope or the amount of $15.00($18.00 for more than two (2) passports) for Express Mail will be collected to cover mailing expenses. Metered stamps will not be accepted . THE CONSULATE GENERAL ASSUMES NO RESPONSIBILITY FOR ANY LOSS THROUGH THE MAIL.
