No appointment necessary. First 125 students per session

Immunizations provided free of charge by Monroe County Department of Public Health for those covered by Medicaid or Child Health Plus/Family Health Plus, students without health insurance or adequate coverage and Native Americans. Please bring your immunization records. All students must be accompanied by a parent or legal guardian.

NYSED requires an ANNUAL physical exam for new entrants, students in Grades K or 1, 2, 4, 7 and 10, sports, work permits, and triennially for the Committee on Special Education (CSE).

There are NO EXCEPTIONS!

A parent or guardian must accompany the student or the student must bring a signed parental consent form, which can be obtained at the RCSD website at http://www.rcsdk12.org. Click on the Health Services quick link. The form is “Physical Examination; Parent Consent Form.”

As a part of the comprehensive physical examination needed for school, sports, and work permit compliance, participants in the free physicals will have evaluations of their vision, body mass index (BMI), height/weight and if necessary a mental health screening, EKG/ECG and immunizations. Adolescents ages 11-19 will complete a risk assessment survey (without assistance/participation from parents/guardians) and have counseling related to safety and prevention of risky behavior. The risk assessment survey and counseling are required parts of the free physicals for adolescents ages 11 to 19. Requests for partial physicals and/or physicals without participation in the risk assessment survey and counseling will not be honored.
