  38. Attendance classroom training (including practicum session with observation of daily and terminal room cleaning)
  39. Pass written exam on classroom content
  40. Pass 2 daily clean observations conducted by site EVS manager or designee within 4 weeks of classroom training.
  41. Pass two UV gel assessment of terminal clean with minimum 80% success on high touch surfaces within 4 weeks of classroom training
  42. Continued observed audits of practice
  43. Direct observation: Each employee will be observed cleaning a discharge and daily clean once yearly.
  44. UV gel testing: Each employee will have a minimum of one room checked annually.
  45. Re-training
  46. Employees who fail audits of practice will receive retraining.
  47. Continued failure to meet requirements will lead to corrective action.
  48. Annual Training and Recertification
  49. Employees will receive annual training that will include review of basic cleaning concepts and ongoing issues identified through observational and UV testing audits of practice.
  50. Successful attendance at training session and written exam is required for recertification.
