Directions: The candidate is responsible for bringing this form to the mentor teacher in advance of its due date. The candidate should wait for mentor to complete the form and participate in any discussion and reflection associated with it. After the mentor has completed the form, it is the candidate’s responsibility to upload it to Blackboard by the deadline communicated by the Field Coordinator.
