Below is a listing of documents/informationrequiredand acceptedby the Early Learning Coalition of NW Florida, Inc for eligibility determination.
Verification of All Family Earned Income for household members included in your family size.
6 weeks of current, consecutive pay stubs – Must have company name, parent/guardian name, and #of hours worked
Complete currentincome verification form
Tips – Reported and unreported (Self Attestation)
Self-employment verification (IRS Tax Schedule C,1099’s, accounting records, contracts, receipts, Casual Labor - Work Calendar showing dates worked, for whom, amount received.) Expenses must be verifiable with receipts of payment
Any and all earned income and working hours must be verifiable.
School Scheduleor document completed by an official of the school/institute showing:
Number of classroom hours & lab hoursDate the semester/training starts and ends
Verification of all other income
Child Support – Clerk of Court website, Notarized statement from absent parent with date, address, phone number, & amounts paid for the last 6 weeks.
Social Security – Award letter (updated in January) for SSA and/or SSB (Survivor’s Benefits) – If SS is received because of a disability a note from a physician confirming disability and stating the need for childcare assistance
Alimony – Court order
Unemployment- Form UCB-11
Relative Caregiver – Written documentation by DCF or current award letter
TANF –Award letter
Verification of Age - Verification of Age for all children in the household. – Birth Certificate
Proof of Legal Guardianship (if applicable)
Court documents indicating guardianship
Proof of Participation in Relative caregiver program
Award letter of Payee status of SS benefits
Notarized statement from parent(s) – Statement requirements - Anticipated time frame, parent’s name, address, telephone #, andanother form of corroborating evidence such as: filing for child support or verification of the parent’s absence (incarceration or treatment facility)
Social Security Cards (optional)
Proof of Marital Status (if applicable)
Dissolution of Marriage
Notarized statement from absent parent with date, address, & phone number
Current Florida Driver’s License or Government issued Picture ID – ID cannot be expired
Proof of Residency
Current Driver’s License
Current Utility Bill – electric, water, cable, landline telephone
Current Pay stub
Residential rental agreement – signed by landowner
Voter’s Registration Card
Choice of Provider selected prior to the appointment to enroll
Eligibility Criteria for School Readiness Services
Single parent family – Parent/guardian must be employed a minimum of 20 hours per week or participating in an eligible education activity full time or a combination of both.
Two-parent family – Both parents/guardians must be employed a minimum of 20 hours per week or participating in an eligible education activity full time or a combination of both.
Must be a resident of Bay, Calhoun, Franklin, Gulf, Holmes, Jackson, or Washington County.
