Case Summary: Include the following information to summarize the case
  • The start of the period under review, case type
  • The date the child came into care and the date the case closed (when applicable)
  • Reason for case opening/removal, such as physical abuse, general neglect, etc.
  • Permanency goals in place under the PUR (include dates necessary, i.e. date a goal changed, date of TPR, date a child returned home, etc.)
  • Family constellation: identify target child (if applicable) by age/DOB, age/DOB & number of siblings along with where they are now, who lived in the home at time of removal, and other pertinent family members for this child. Include whether parents live in or out of the home with the child. Paramours and other adults that have any continuous contact or caretaking role with children are identified and described. Include any relevant information about target child or family members (i.e. developmental delays, medical conditions, etc.)
  • Number of placements for out of home cases, dates in the placements
  • Explain why participants were not interviewed, including whether an inquiry was submitted to CDSS and the outcome of that
  • Total number of social workers assigned with dates of assignments
  • Any other information that is background info that is needed throughout the case (i.e. unusual circumstances, parent whereabouts, safety threats, etc.)
