Dear Provider:
Thank you for expressing an interest in becoming a Voluntarily Registered Provider. Voluntary Registration is a form of regulation set up for providers who wish to keep no more than five children at any one time, in the Provider’s home. Please take the time to read this letter in full.
The requirements for becoming Voluntary Registered are:
1. Retrieve the initial Voluntary Registration Packet from our Website www.childsavers.org. The initial packet includes the following:
· Application
· Criminal Record and Sex Offender ($20 per person) must be submitted to Virginia State Police. These reports must be completed and dated within 90 days from turning in the application. NO EXCEPTIONS
· Sworn Statement of Affirmation
· TB skin test results or TB screening dated results within 90 days of turning in the application. NO EXCEPTIONS
· Health and Safety Checklist (should be completed prior to health and safety visit).
· Voluntary Registration Knowledge Assessment (this is not a test, there are no right or wrong answers)
· All persons 14 years and older living in the home, assistants and substitutes are required to have a child protective services clearance ($10.00PER PER PERSON).
2. Turn in a completed Voluntary Registration Packet to ChildSavers; this includes all of the items listed in step 1. It’s best to submit paperwork all at the same time. Remember to include the following when submitting your packet:
· A $50 Non-Refundable application fee is due when turning in your initial packet. Packets submitted without the application fee will not be processed, NO EXCEPTIONS
· Fees for all backgrounds are paid by the provider and are not included in the $50.00 application fee.
· All of the above information must be original, current and the processing dates must be within 90 days of the date you turn in the completed application packet.
3. A home visit will be scheduled when a completed Voluntary Registration packet is received by ChildSavers. This visit is to verify compliance with the health and safety checklist. Please keep the checklist until the home visit. A certificate of registration will be recommended once all requirements on the Health and Safety Checklist are met.
All requirements must be met within 90 days from the date we receive your application. ChildSavers will only process completed and current application packets. If an application is not complete or out of date, the information will be returned to the provider. After 90 days your file will be closed if no completed. If you wish to proceed, all fees and paperwork will need to be completed again. Packets should be mailed to: Voluntary Registration 200 N 22nd Street Richmond, VA 23223.
