Parent Checklist for Washington Paperwork:
Student Information Form: Blue Form;Return tonight. Fill out completely/Legibly.
- When writing e-mail addresses:
- PLEASEPRINT
- If the email address is case sensitive, indicate so.
- Be sure to put a line through a zero and leave the letter “O” open
- One email address will be used unless otherwise requested
Parental Permission Form: Back of Blue Form;Return tonight.
- List an email address that is checked daily
- Please read and have your child read the Code of Behavior
- The Code of Behavior needs to be signed by both a parent and student.
Protocol for Self-Administration of Medication Form:Green Form; Return tonight.
- Please read this thoroughly with your child
- Both a parent and the student need to sign this form
Hold Harmless Form “AGREEMENT”: Back of Green Form;Return tonight
Student Emergency Information Form: Yellow Form; Do Not Return.
- Fill out thoroughly
- Should not be returned, should be keep on student’s person at all times while in DC
MEDICATION FORMS: White Forms; should be returned within a month of the trip.
Off-Site Medication Form for Non-Prescription Meds: only return if bringing non-prescription medications on trip:
- Use this form for any over the counter medications you will sent with your child (i.e.: Advil, cough drops, Tylenol, Benadryl, Anti-Acids, etc.)
- Be sure your child knows how to take the medications you send
- Medications must come in original bottle
- Form doesn’t need to be returned if medications are being sent
Off-Site Medication Form for Prescription Meds: only return if bringing prescription medications on trip:
- Use this form for any prescription medications your child will take in DC
- Student’s MD must sign this form if the nurse does not have a med sheet on file
- If your child has an Epi-Pen, Inhaler, etc. in school and you and your MD have signed a Medication Form, then complete this form without the MD signature.
