Job title:Community Champion

Reporting to: Senior Community Champion

Staff: None

Main purpose:

  • Toworkwith colleagues and volunteers to raise awareness and understanding of the liver and liver health within the general public.
  • To offer support and information to those affected by all forms of liver disease, through our support group network and partnership with local health care professionals.

This job is for 20 hours per week for a fixed term of two years.

Full/clean driving licence and own vehicle is essential.

Job Description:

Specific responsibilities will include:

  1. Maintaining communication with and support existing groups – including promoting the patients’ voice by obtaining feedback through patient surveys etc.
  1. Organise/participate in health events in commercial and larger organisations, eg: universities etc, to raise awareness and understanding of the liver and liver health to staff / students.
  1. Research and utilise opportunities to promote the corporate ‘Love Your Liver’ package within corporate world.
  1. Maintain or build excellent working relationships with liver teams in region, to ensure that lines of communication are strong and beneficial to patients, health care professionals, third sector and others.
  1. Organise/participate in health events within primary and secondary care to raise awareness and understanding of the liver and liver health to patients, carers and health care professionals, as well as to the general public.
  1. Representing the Trust and advocating for people at risk of or affected by liver disease at local and national meetingseg: the development of a Welsh Liver Plan with the support of local patients.
  1. Organise, participate in and evaluate a series of health events within a designated region as determined by prevalence of Liver Disease within given region.
  1. Help to identify potential leads/relationships within commercial organisations, health care services and educational establishmentsthat may be interested in awareness raising events.
  1. Assisting in the formation of new support groups; where there is a clear demand and available resource from health care professionals in particular.
  1. Identify individuals who have had liver conditions and who would potentially benefit others by sharing their story.
  1. Update the trusts database on a regular basis and keep personal and team calendars updated at all times.
