1. Main Purpose of Job
To carry out high quality holistic, outcome focussed, person centred care and support to individuals accessing Sue Ryder Services
2. Position in Organisation
To work within the Health and Social Care Team. This position carries no line management responsibilities. The role reports directly to Care Co-Ordinators.
3. Scope of Job
Clinical / Technical Responsibility: Carry out delegated care and support tasks whilst ensuring a safe, clean and comfortable environment
Clinical Governance / Quality: Ensure that the rights and choices of individuals are actively promoted and supported and their beliefs and preferences are appropriately acknowledged
Managerial Responsibilities: To comply with national and local policy , procedures, guidelines and legislation
Training and Development: Completion of local and corporate induction and attendance of mandatory and priority training
4. Dimensions and limits of Authority
This post holds no budgetary or managerial responsibility
The post holder is expected to be mindful of property and resources
5. Qualifications
S/NVQ /QCF Level 2 or equivalent or be willing to complete
6. Experience and Skills
- Numerate and literate
- Good communication skills
- Experience of working within a team
- Effective time management skills essential
- Empathetic and sensitive approach essential
- Basic IT skills awareness including accessing the internet, email and word desirable
- Full Driving Licence and access to car for use at work with valid business insurance
7. Duties and Key Responsibilities
Clinical / Technical Responsibilities(must reflect clinical risk assessment)
- To promote choice in the delivery of care and support. Reviewing and making suggestions to enhance the individuals’ experience and enabling Service User to achieve desired outcome. Enabling concerns and complaints to be raised freely by the individual and/or their representatives
- To ensure accurate reporting and documentation
- To support service users to have their views and experiences taken into account in the way the service is provided and delivered in order to capture wishes and aspirations, promote independence and achieve desired outcomes
- To participate in care planning implementation and review as a core team member
- To promote the values, build morale and encourage others to perform to high standards

B Clinical Governance / Quality
- To promote an organisational culture where staff are empowered to challenge inappropriate behaviour and unsafe practice using appropriate organisational processes including the internal whistle blowing policy
- To report all incidents and concerns in a timely manner
- To be an active and co-operative team member. Challenging practice as appropriate and supporting new staff
- To participate openly and honestly regarding care delivery
- To participate openly and honestly in any investigation if care falls below expected standards
- To meet the requirements of the post holders terms and conditions of employment
- The post holder will ensure that they attend mandatory and priority training as required
- Further competencies will be identified through personal development planning, appraisal and supervision
8. General
