The New York State Health Facilities Association (NYSHFA) is a statewide membership organization of nearly 300 nursing homes in NYS. The website is http://www.nyshfa.org/ for NYSHFA.
The New York State Department of Health Nursing Home Information (NYSDOH NH Info) website is: http://www.health.state.ny.us/nysdoh/consumer/nursing/homenurs.htm. The NYS Health Department website http://www.health.state.ny.us/nysdoh/nursing/main.htm provides tips on finding a nursing home in New York State as well as the summary reports of state surveys conducted every 15 months to investigate the quality of care and life of residents.
The Medicare website also offers a way to compare local nursing homes. Nursing Home Compare is a tool to provide detailed information about the performance of every Medicare and Medicaid certified nursing home in the U.S. This website now includes the health deficiencies that have been cited as a result of investigations after complaints were filed.

ALZHEIMER’S /
DEMENTIA: Facilities may choose to include individuals diagnosed with Alzheimer’s disease and/or other dementias in their general population, or may have separate units to provide long term care.
REHABILITATION: Some facilities offer a separate rehabilitation “wing” or “unit.”
Note: REHABILITATION (REHAB) is defined as: “What it takes (therapies, equipment, environmental modifications, and other resources) for someone to achieve maximal functional independence within their environment. This includes: Physical therapy (PT), occupational therapy (OT), speech therapy (ST), vision rehab, psych rehab, driving services, music therapy, prosthetics, equipment vendors, and access equipment.” Settings for these services include: Acute care facilities (hospitals), sub-acute care facilities, nursing homes (even those without “designated beds”), outpatient services, day treatment programs, in-home health care.
RESPITE /
SHORT TERM CARE: Some facilities offer, on availability, a bed for a short period of time. Facilities that have New York State designated respite beds must reserve one or 2 beds specifically for respite patients. A Patient Review Instrument (PRI) and Screen may be required prior to admission. A PRI and Screen is a form completed by a qualified person that rates the person's ability for self care.
